PERMIT TO WORK PROCEDURES

Many operations require a Permit-to-Work (PTW) System. These include such operations as, Excavations, Entry
in to Confined Spaces, Demolition, Hot Works, Working in Riser Shafts, Electrical Work, etc, of which the purpose
is to protect the operatives whilst working in a potentially dangerous environment.

A Permit-to-Work system is a formal written system used to control certain types of work that are potentially
hazardous. It is a document which specifies the work to be done and more importantly, the precautions to be
taken. It lays out the a safe sequence of work, which if not followed, could lead to a dangerous situation.

A good Permit-to-Work Procedure ensures that only authorised personnel are allowed to enter designated areas
and carry out works within a certain environment and that those persons are specially trained. It ensures that only
specialist equipment, tools and materials are used and that any dangers out of their control are minimised
elsewhere in the system.

These procedures are used when normal safeguards are dropped and risks are present. It is also a clear record
that all foreseeable hazards have been considered.

Too many accidents occur because operatives take it upon themselves to change the work sequence to suit
themselves or totally ignore the safety precautions specified on the permit.

IT IS IMPORTANT YOU ENSURE THAT:

 You know and understand your copy of the procedures set out in the Permit to Work.

 You understand the importance in following these procedures, as you may not be aware of other hazards
outside your control.

 Never enter into a situation where you do not have a safe procedure in place.

 Follow the exact procedure and if any situation arises that could change the work situation TELL YOUR
FOREMAN.

 Never skip, re-arrange or ignore procedures as this could cost SOMEONE ELSE their life.

 You have the training and experience to carry out the work safely, if it is outside your experience, STOP and
report to your foreman.

 Only use the designated tools and equipment specified on the Permit.

 Understand the contents of the permit, if you don’t, TELL YOUR FOREMAN.
 Check if the Permit is valid and authorised, before you start work.

 You have a permit in your possession at all times.

 Understand the requirements of the permit system and never allow others to put YOU at risk.

 Never work outside the designated period of the permit as this could hold up other essential works and/or
possibly put others at risk. Remember, this is a Safe System of Work

The Permit needs to contain the following information:

 Permit title

 Permit No

 Job location

 Plant Identification

 Description of works to be
done and its limitations

 Hazard Identification

 Precautions necessary

 Protective equipment

 Authorisation

 Acceptance

 Extension/shift handover
procedure

 Hand back

 Cancellation
