CONSENT LETTER

Dear Sir/Ma'am,

Greetings!

The Performance Task: Zumba in the subject of Physical Education. We would like to hold a
practice on December 9, 2022 Friday and December 10, 2022 Saturday outside the school
premises at the time 9:00am to 4:00pm only. It will be held in Roseville Subdivision, Amuntay
Road Zone 3 Dasmarinas Cavite ClubHouse. Due to unforeseen circumstances we couldn't
practice at school in some sort of conflict in schedule so we will hold this said practice.
Furthermore we will collect an amount of 35php for every student of J.Rizal Batch1 because of
the shortage in budget we had. We assure you that the amount we gathered will be only used in
the unpaid printing of the Backdrop for our theatrical play and for the other props only. Thus this
letter will authorize your child to follow strictly what has been stated above the letter.
