    Tier 1 MERP equipment

    Description:
    It was noticed during the Audit that valid and in-date Tier 1 equipment was
    not available in the photographic evidence sent to OPITO prior to the audit
    Root Cause Analysis:
    WHY 1:
    There was a mix-up, as the Tier 1 first aid equipment & supplies photographic
    evidence sent to OPITO for review during the audit was an old photographic
    evidence similar to the one sent during the initial site visit of June 2021.

    WHY 2:
    Different/ multi-persons where assigned to take/send in photographic evidence
    to upload to OPITO as evidence with no one delegated to review the
    photographic evidence for accuracy, validity and clarity.

    Corrective action:
    1) The training coordinator has been assigned with the sole responsibility
    of taking photographic & video evidence whenever it is required by
    OPITO.
    2) The document controller has been delegated with the responsibility to
    always review all photographic & video evidence for accuracy, validity &
    clarity before sending to OPITO as evidence to meet OPITO
    requirements.
    3) An Excel spread sheet shall be put in place using conditional formatting
    to alert the Tier 1 designated first aiders whenever first aid equipment
    that has expiry is due.
