AFFIDAVIT OF CONSENT
(TO THE ADOPTION)

I, _________________, Filipino Citizen, born on June 24, 2006, and with

address at ________________Baguio City, after having been duly sworn to in
accordance with law, hereby depose and state that:

1. I am the second child of____________;

2. I never met my biological or my real father neither do I know

anything about him or his whereabouts;

3. I grew up with my grandparents, _______________, they are the ones

who provide for my food, clothings and the ones who send me to
school;

4. _______________is my third child with yet another boyfriend,

_______________and just like my second child, the biological father
was the one who gave information to the clerk for the entries in
his Certificate of Live Birth but just like my failed relationships,
we ended the same fate and I was left with the custody of my
children with my parents primarily providing for us since I was
then unemployed;

5. Years passed and we all lived under the care and support of my
parents. I would be employed for contractual engagements and
would then again

6. My parents were the ones who supported me financially since I

got pregnant with Paulo because I was then still studying;

7. It was also my parents who have been providing for my children

and treated them as their own children since my only brother
died;

8. I have witnessed how my parents love and care for my chidren

continually throughout their growing years up to now. I know
and fully believe that my parents can and will give all the love,
care and support to my children;

9. Considering all the foregoing, all for the benefit of my children, I

have decided that it is for the best interest and well-being of my
children that I agree with my parent’s intention of adopting them;
 10. I hereby give my unqualified and unconditional consent to the
adoption of my children, _____________by my parents
_______________solely for the best welfare of my children, fully aware
of its legal consequences under the provisions of the Family Code
and related laws and the Rules on adoption; and

11. I am executing this Affidavit of Consent for the purpose of

attesting to and confirming the veracity of the foregoing facts and
declarations, in support of the Petition to be filed by my parents.
