I understand that my employment at or its affiliated companies is subject to positive responses from my references,
bonding eligibility and continued adherence to and its affiliated company’s policies and procedures, applicable rules
and regulations.

I consent to take any pre or post-employment examinations as may be required by and its affiliated companies or its
representative, and release and its affiliated companies from any liability that may arise from such examination.

I understand that as is the process followed by most of the police departments across India for criminal background
verification, it is possible that the police authorities may contact you or visit your stated residence and at times even
ask you to present yourself at the concerned police station. It is part of the standard verification procedure. In case of
any attempts of coercion by the authorities and in the event of any occurrence, I would notify our respective human
resource officials immediately.

I authorize an inquiry to be made on the information contained on this application. Former employers, officials of
education institutes, government bodies and financial institutes are authorized to give information about me to
authenticate the information provided on this application and I release them from all liability for issuing such
information.

I hereby attest and warrant that all my answers on this application as well as on all forms completed in conjunction
with my employment are true and accurate. I understand that my misrepresentation of facts, failure to disclose
information required on this application or material change in my information provided which is not reported to
Human Resources shall be cause for dismissal regardless of when discovered by and its affiliated companies.
