PARENTAL CONSENT

I confirm that I Marlene N. Rivarez, the parent/legal guardian of Fernalyne R. Abila hereby
consent and permit my child to attend the Rewards and Incentives for Students, Parents, and
Stakeholders at Camarines Norte Senior High School Covered Court, on June 29, 2022, 1:00 PM
to 7:00 PM. I understand that this event would certainly bring happiness and satisfaction to
anyone who has overcome the trials of the new normal.

I acknowledge that my child and all the people involved are going to follow the COVID-19
IATF Guidelines and health protocols to ensure a safe conduct of their activity. I also
acknowledge that no harm or injury will be inflicted on my child upon participating in this event
and I make certain that I can pick up my child and ensure his/her safe return home. I confirm that
all details are correct and I am able to give parental consent for my child.
