participate in its face-to-face classes and activities.

I am aware that their face-to-face classes and activities will be conducted in enclosed
spaces like classrooms and that the areas where these classes and activities will be held will
utilize up to 100% of their seating or carrying capacity.

I am equally aware of the risks involved and the possibility of Covid-19 exposure and
infection in these areas.

In view of the foregoing, I release Taclobo National High School, its officers and
employees from any and all liabilities arising from Covid-19 exposures and infections that may
occur in the course of my child/ward’s studies in Taclobo National High School.
