AFFIDAVIT

I, MATTHEW FRITZRAYMOND WOLFF, Filipino, of legal age, married,

with postal address at L 27 B 38 Kamia St, Milflora Homes, Sabang,
Baliuag, Bulacan, after having been sworn to in accordance with the law,
depose and state, that:

1. I am a holder of Non-Professional Driver’s License No. C03-15-

444481 valid until April 20,2028 .Copy of which is hereto
attached as Annex “A”;

2. I am the registered owner of a Toyota Innova with plate number

VU 2264. Copy of the Official Receipt and Certificate of
Registration of my Toyota Innova are hereto attached as Annex
“B”;

3. On September 5, 2022 around 3:00 P.M. I was driving my

Toyota Innova with plate number VU 2264 at North Luzon
Expressway , a truck in front of my vehicle was also traversing
the same road.

4. All of a sudden, the wheel of the truck in front has accidentally

hit a fist-size rock, which caused the rock to be hurled towards
my direction, hitting my vehicle’s windshield. My windshield
shattered due to the impact. Picture of the damage of the
vehicle are hereto attached as Annex “E”.

5. I am executing this affidavit to attest to the truth of the

foregoing facts and for any legal purpose it may serve best.
