INVESTMENT LINKED POLICY


If your policy is investment-linked, this is just to inform you of an unsuccessful Credit Card deduction
attempt on the date stated overleaf for the premium due under your policy. We will continue to deduct
the premium from your Credit Card for as long as your policy continues to be in force.

NON-INVESTMENT LINKED POLICY: WITHOUT SURRENDER VALUE


If your policy has not acquired a surrender value, it will lapse if the stated premium is not settled. We
therefore look forward to receiving from you this premium before the expiry of the days of grace for its
payment, and it should be paid by cash or cheque sent directly to the Company.

NON-INVESTMENT LINKED POLICY: WITH SURRENDER VALUE


If your policy has acquired a surrender value, an Automatic Premium Loan will be raised to settle the
stated premium, after the expiry of days of grace for its payment.

TO RESUME CREDIT CARD SERVICE USING THIS BANK ACCOUNT


After your premium is settled either by (a) direct payment to the Company by cash or cheque; or (b)
Automatic Premium Loan, the Credit Card deduction will automatically resume from the following
premium, even if your policy has Automatic Premium Loan outstanding.

TO CANCEL CREDIT CARD SERVICE


If you decide to cancel the existing Credit Card service, please notify the Company in writing.

IF THIS CREDIT CARD IS NO LONGER APPLICABLE FOR CREDIT CARD SERVICE UNDER
THIS POLICY
To reapply for Credit Card service using a new credit card, please submit a fresh Credit Card
Application Form and deduction will commence from the premium due after the application has been
approved.

GENERAL INFORMATION
The date the Company has arranged with the Bank to process all our Credit Card deductions will
follow the date in which your premiums are due.
