Please ensure that the new card number is correctly registered. We advise that you check your card statements
regularly to ensure continuous coverage of your insurance policy. If you suspect any unauthorized premium
deductions had been made to your account or other irregularities concerning your premium deductions, you should
notify your bank and Great Eastern Life Assurance (Malaysia) Berhad immediately.

THIS POLICY
To reapply for Credit Card service using a new credit card, please submit a fresh Credit Card
Application Form and deduction will commence from the premium due after the application has been
approved.
GENERAL INFORMATION
The date the Company has arranged with the Bank to process all our Credit Card deductions will
follow the date in which your premiums are due.
