Title: Cable pulling work on road Manhole,
In-building and External,
1. Cable pulling work on road Manhole:
a) Adequate Safety provisions are to be standby in the transport lorry such as safety cone,
plastic barricade, and confined space warning signboards, advance warning signboards
1. Caution 2. Works Ahead 3. Temporary road lane diversion and road lane diversion
arrows and sign board stands, blinking lights/rotating lights, multi gas monitors and
appropriate manhole keys.

b) Adequate rescue equipment’s such as rescue Tripod, self-contained breathing


apparatus (SCBA) and Medical oxygen resuscitator sets are to be standby in the
transport lorry for emergency purpose.

c) Serviceable generator, water pump and exhaust fan and air duct must be available and
be in good working condition.

d) Supervisor / foreman ensures that fuel refilling the water pump, generator done before
go to work site, at the fuel station, it must not be overfilled pumping / topping the
fuel must be carry out by the supervisor/foreman and not the pump attendant.

e) Use safety petrol can for storing of petrol and any fuel i.e. petrol, diesel, corrosion
spare tanks are only allowed to store on metal oil tray to prevent spillage.

f) Once the transport lorry / vehicle reaches to the work site traffic controller should
divert the vehicles from manhole lane to the next lane.

g) Close the road lane and place adequate Safety cones, provide plastic barricade around
the manhole display sign boards (confined space / no entry) onto one of the plastic
barricade column and to place advance warning signboards such as 1. Caution with
rotating light, 2. Works ahead with Contractor logo and 3. Temporary Road lane
diversion at before the work zone and provide work zone arrow signboards with
rotating / blinking lights at before three (3) meter of work zone all this safety
provisions are to be provided accordance to LTA Code of Practice for temporary Traffic
Control before proceeding to open manhole to demarcate the worksite / manhole to
warn persons of the existence of the manhole.

h) Supervisors and foreman that whenever on road manhole works is carried out it is
essential and necessary that all personnel must inform necessary personnel to get
Truck Mounted Attenuator (TMA) lorry to be parked in front of the work zone if major
roads are involved i.e. road speed limit greater than >70km/hr.

i) Manhole must not be opened for any reasons if S/No- g and h was not done.

j) Not obstructing the traffic and public while control traffic or placing safety provisions.

k) Site supervisor to make application the PTW and indicate if any precautions taken.

l) Open up the manhole cover by using the manhole key (normal manhole key / 4pcs
manhole cover key) only after the area was properly barricaded and displayed of
confined space warning signboards.

m) Never try to open up any manhole cover with manhole cover key if the manhole key
hole is found damage and manhole key cannot fit inside such case must report to NLT
immediately for repaired action to be carried out on damage manhole key hole before
work can be carried out.

n) Manhole Safety Assessor to do the gas check on the manhole by using the multi gas
meter.

o) Manhole Safety Assessor to place down sample tube onto the manhole to do gas check
1st readings check of the 02, PEL, CO and LEL level and record results in entry permit.

p) Pump the manhole water out completely before conduct the gas check on the manhole.
2nd reading.

q) Diesel or any other liquid mixed with other harmful substance inside the manhole is
seen, do not pumps out the water to the public or any other drainage as this cause
violation promptly inform necessary person and seek for NLT officer approval to
arrange oil tanker lorry to remove that oily (diesel) water from manhole before
starting work.

r) Place the air duct in the manhole to blow away the existing gases and put the water
pump with hose onto the manhole and open up the side road drainage cover and place
the side of the hose onto the side drainage to drain out the water.

s) Effective ventilation must be ensured especially for deep manhole and for those
situated near petrol or gas storage areas e.g. Petrol Kiosk, Petroleum Refinery, Gas
Factory or Vehicular Workshops.

t) Manhole Safety Assessor to place down the gas meter sample tube onto the manhole to
do 2nd reading checks of the 02, PEL, CO and LEL level and record results in entry
permit.

u) Setup the rescue tripod stand for all manholes that is more than 1.5 meter deep and
ready for entrants to go down the manhole.

v) Made available the SCBA and oxygen resuscitator sets onsite for emergency purpose.

w) Manhole Safety Assessor assess all precautions are taken and endorse the application if
he satisfied that the work can be carried out safely and forward it to the authorised
manager.

x) The authorised manager may issue a PTW if he is satisfied it is safe to carry out such
work.

y) The permit to work [PTW] form has to be displayed at entry of the manhole.

z) Manhole Attendant standby at entrance of the manhole to monitor the manhole entrant
when entering and working in a manhole / confined space.

aa) Place the ladder down onto the manhole for manhole entrant access.

bb) Entrants to wear rescue harness connected with rescue tripod lifeline (stainless steel
wire rope) and go down the manhole. Entrant must be equipped with personnel gas
meter at all time when entering and working in any manhole.

cc) The gas meter must be serviceable and it must be calibrated once a three months and
have a valid certificate.

dd) Turn the instrument on by holding the ON/OFF/MODE (larger button) for 3 seconds and
then releasing. Instrument will turn on, show all icons, and check battery to ensure
charge before use. After it runs through a series of screens it will go into a countdown.
It will then stop at the real-time.

ee) Instrument should be Zeroed in fresh air (no background gas levels). Press the
ON/OFF/MODE button multiple times to get to the Zero Initiate Screen, then press
ENTER (smaller button on right) to start.

ff) Gas detectors must be fully calibrated once every three (3) months. Bump-test is to be
done on a monthly basis, within this period, if the gas detector is found to trigger the
calibration/bump test warning alarm or if suspected to be faulty, the respective Site
Supervisor or Manhole Safety Assessor is to be send it to the authorized personnel for
assessment. If necessary the gas detector has to be re-calibrated, or bump tested prior
to use.

gg) After a bump test, peak readings should be cleared. Press the ON/OFF/MODE button
multiple times until reaching the Peak Readings Screen. Press the ENTER button once
to clear the peak values.

hh) The check mark indicates the instrument (gas detector) is monitoring for gas. When it
appears without any service or alarm indicators, the unit has no faults and needs no
service then proceed to use it.

ii) If any part(s) of the gas detector is found to be faulty/damaged/malfunctioning, it is


not to be used.

jj) Instrument should be zeroed in fresh air after a bump test. Peak readings should be
cleared. Refer to the user manual for zeroing procedure.

kk) A bump/function test is to be performed on the monitor once every month. This is
defined as a brief exposure of the monitor to a known concentration of target gases to
verify sensor and alarm function. If the instrument fails to operate properly in a bump
test, a full calibration should be performed prior to use.

ll) All the manhole opened must be in-charge by neither Supervisor, Assistance Supervisor,
Manhole Safety Assessor.

mm) The maximum number of manholes opened at one time is only three (3) manhole.

nn) Upon confirmation and approval to install the fibre cable, the cable will be pre-cut and
deliver to site for installation.

oo) Contractor will change pass / register before proceeding with work if necessary.

pp) No excavation or drilling work is involved, only opening of telecom manhole / riser /
duct / cable tray.

qq) Existing services will not be affected during this cable installation work.

rr) Very minimal noise level will be maintained all through the work.

ss) Entering the manhole using a ladder should maintain three (3) point contact and buddy
system method.

tt) To place the conduit rod, stand on rough / level / stable ground.

uu) Place the conduit rod stand (cable laying equipment) at beside the first (1 st) manhole
and deploy one (1) worker to unwinding the conduit rod (Rotan) and pass it to another
worker (manhole entrant in the manhole) and manhole entrant insert the Rotan head
into the existing NLT PVC pipe in the manhole and push it firmly until the Rotan head
reaches next manhole.

vv) Once the Rotan reaches the next (2nd) manhole the manhole entrant (worker in the 2nd
manhole) tie the Rotan head with cable head.

ww) After worker tie the Rotan head with cable head gives signal to the worker which
worker are waiting at first (1st) manhole.

xx) After signal given the worker at first (1st) manhole start to wind the Rotan until he
saw the cable head.

yy) Once the worker saw the cable head untie the cable head from Rotan head.

zz) After untie the cable head from Rotan head manually pull the cable and coil beside
the manhole.

aaa) Use the above mentioned (S/No-uu to S/No-zz) method to pull the cable according to
the SLD all the way to the customer end and.

bbb) All the workers will be equipped with proper uniforms and PPE i.e. Safety Helmet,
Safety Boots, and Hand gloves to handling cable pulling work.

ccc) Always made available the emergency rescue equipment’s onsite such as first aid box
with adequate content, and flame proof light in vehicle which vehicle is involving
works.

ddd) Hauling sock is not required as the work involve pulling light fibre optic cables where
only ‘man work’ (no machinery) is involve.

eee) There will be roller bar placed over manhole to protect the cable from manhole edge
during cable pulling.

fff) Poly water or similar product (non-oil base) can be used as lubricants during cable
pulling/pushing and recovering. Diesel must never be used as this will act as a fuel
should a heat source be introduce elevating the risk of a fire outbreak.

ggg) The start points of the cable pulling work is in the middle of the cable.

2. Cable end sealing cap procedure:
a) After pulling cable, check the cable for water seepage.

b) Upon confirmation water seepage, Seal the cable with heat shrinkable end cap.

c) Unscrew the valve from the end cap.

d) Cross cut the cable jacket for better adhesion.

e) Insert end cap and applied heat, use wet cloth to cool down the hot melted adhesive.

f) Re-insert the valve into end cap.

g) Provide the tape on end cap and cable jacket.

h) Pump the air 50kpa -60kpa into the cable and check the pressure.

i) Use soap-foam water to check for leaking.

j) Apply tape to protect the value, Tie the cable up right inside manhole shaft.

k) Take photo and close the manhole.

3. Cable pulling work in-building & External:
a) A survey is being carried out as per SLD customer end to determine the cable route is
either from MDF / TER room or outside source at which underground pipes are laid
from the last NLT manhole into the building premises.

b) A proposal showing the cable routing from point to point will be prepared and
submitted to the Building Management / Owner for approval.

c) The route proposed tentatively will be using existing IT / Telecom infrastructure from
Communication / MDF / TER room if any telecom risers, and run along ceiling exposed
or above false ceiling panels.

d) At times, cable is laid under floor system if necessary to reach the termination point.

e) Upon confirmation and approval to install the fibre cable, the cable will be pre-cut and
deliver to site for installation.

f) Place the conduit rod stand (cable laying equipment) at customer lead-in-end pipe /
MDF room / riser room / Ter room / Telecom room and deploy one (1) worker to
unwinding the conduit rod (Rotan) and pass it to another worker and another worker
insert the Rotan head into the existing NLT PVC pipe and push until the Rotan head
reaches last manhole.

g) Once the Rotan reaches the last manhole the manhole entrant (worker in the manhole)
tie the Rotan head with cable head.

h) After worker tie the Rotan head with cable head and gives signal to the worker which
worker are waiting at customer lead -in- pipe end.

i) After signal given the worker at customer lead -in- pipe end start to wind the Rotan
until he saw the cable head.

j) Once the worker saw the cable head untie the cable head from Rotan head.

k) After untie the cable head from Rotan head manually pull the cable and coil beside the
customer lead -in- pipe end start.

l) Use “A” frame ladder to access the vertical and overhead cable tray to maintain three
(3) point contact and buddy system.

m) Deploy one (1) worker to unwinding the coiled cable and pass it to the person who
standing on the “A” frame ladder and lay the cable on the cable tray according to the
Straight-Line Drawing / scheme (SLD) given by NLT.

n) Use the same methods S/No- k, l and m to pull the cable to each level of the building if
required by NLT.

o) Condition 1 – Only indoor cables are pulled from Riser / TER room to customer end at
which outdoor cable are terminated at Riser / TER room.
1) Tentatively, the cable will be placed at mid span of the route, one end shall be
pulled to MDF room / Ter Room.
2) Existing IT infrastructure system – cable on tray / Trunking will be used for cabling
works from end to end.

p) Condition two (2) – outdoor cable will be pulled directly from outside source to NLT
required point.
1) Cable will be pulled through existing telecom pipe / duct / underground telecom
manhole PVC pipe, telecom vertical / overhead cable tray to MDF room or telecom
riser to the building.
2) From customer lead in pipe end, cable will be laid through existing Telecoms cable
tray / duct.

q) Pump the manhole water out before dismantle MCT / duct seal to prevent manhole
water leak.

r) Use correct size spanners and hold firmly when dismantle and install the MCT / Duct
seal.

s) Tie the string with spanner and tie on correct reasonably point to prevent spanner from
falling down from users hand.

t) After cable pulling seal up the MCT / duct seal with MCT foam where and when
necessary

u) Monitor the water leak after foaming MCT / duct seal.

v) Supervisor to ensure that no water leak before leave the work site.

w) The cable will be coiled in the server room with slack of 10m to 20m.

x) Each manhole one (1) or two (2) workers deployed for cable losing or cable unwinding
to another manhole or next point of cable laying such as MDF / Riser / Ter room during
pulling cable.

y) Normally the minimum bending radius of the optical cable is 15 times of the od during
operation and 20 times of the od during installation.

z) NLT underground cable shall meet maximum pulling load of 2500n and there is no
specification about pulling speed in international standards.

aa) Supervisor to alert the workers to move to the designated lightning shelter / shelter
that provided with lightning protection / lightning resistances equipment.

bb) There is always two (2) ways communications between either ends during the work.

cc) Eating, drinking and smoking is prohibited during the work especially near the opened
manhole attendant has to inform passer-by not to smoke when passer-by the opened
manhole.

dd) Keep tools in transport lorry after work provide plastic barricade around the work area
and manhole display with warning signboard when there is manhole half close
temporary stop work and lunch break all the portable generators, water pump blower
fan and coiled cables etc.., must be inside the barrier area.

ee) Supervisor to appoint the Banks man to watch over the work location and guide the
Publics or passer-by when not possible to barricade i.e. when there is manhole half
close temporary stop work and lunch break.

4. Cable recovering work:
a) No one is allowed to cut any cables during Recovery if it is not clear, check with the
Supervisor, Check with NLT Site Supervisor if it is not clear, Stop the Recovery Works.

b) NLT shall have their staff to supervise the whole recovery process in addition to the site
supervisor this will ensure a tighter surveillance on the whole recovery process.

c) Site Supervisor and Site Manager must be onsite to assure and verify the recover cable
is marked (by yellow tape) and cut by NLT jointer (Take Photo) and signed the recovery
verification form by all parties.

d) Site Supervisor must be present to supervise/verify during cutting of cable tie or


fastener.

e) Site Supervisor must brief all workers not to cut any cable before the recovery and
record it in the tool box meeting report.

f) For whatever reason that require cable to be cut in the manhole take photos,
i. NLT must be present to cut the cable. If no staff, stop the work and report back to
the in-charge.

g) SOP (Verification Form) for Cable Recovery must comply at all the times of Cable
Recovery works.

h) Coiling of cable out of the manholes. This has to be reported in the daily work
completion report.

i) If duct seal encountered in the pulling operations, report in the daily completion report.

j) When return recovered cable, for cable > 400 pair, cut one meter for store to weigh to
determine the cable length returned.

k) To measure the cable length in the cable chamber with the presence of DO and NLT
staff before the recovery in the cable chamber.

l) To cut the cable at half a meter (0.5m) below the cone block in cable chamber.

5. Use of electrical Powerpack cable cutter:
a) Wear proper hand gloves when using electrical Powerpack cable cutter.

b) Two (2) workers is to cut the cable when using Powerpack cable cutter to cut cable.

c) Only one person are not allowed to cut the cable when using Powerpack cable cutter to
cut cable.

d) Two (2) persons is require to cut the cable when using Powerpack cable cutter to cut
cable.

e) One person operating the Powerpack cable cutter and another one put the cable for
him to cut.

f) Use one hand to hold the cable cutter handle firmly while cutting, using underhand grip
and another hand use the control button.

g) Ensure that both hands are used to cut the cable while using Powerpack cable cutter.

h) User must not use one hand only when using cutter as it is potential hazard to hand cut
(Pinch point).

i) Do not ever clean / touch the Powerpack cable cutter blade while the cutter are
functioning.

j) Cleaning of Powerpack cable cutter blade must be carry out only when all the electric
source are disconnected.

k) Use cotton cloths to clean the Powerpack cable cutter blade regularly and wear hand
gloves when cleaning.

l) Do not use the Powerpack cable cutter while raining it leads the user electrocution.

m) Supervisor to appoint competent person to operate the Powerpack cable cutter.

6. Cable pulling work at night:
a) Close the road lane and place adequate Safety cones, provide plastic barricade around
the manhole display sign boards (confined space / no entry) onto one of the plastic
barricade columns and to place advance warning signboards such as 1. Caution with
rotating light, 2. Works ahead with Contractor logo and 3. Temporary Road lane
diversion at before the work zone and provide work zone arrow signboards with rotating
/ blinking lights at before three (3) meter of work zone all this safety provisions are to
be provided accordance to LTA Code of Practice for temporary Traffic Control before
proceeding to open manhole to demarcate the worksite / manhole to warn persons of
the existence of the manhole.

b) Supervisors and foreman that whenever on road manhole works is carried out it is
essential and necessary that all personnel must inform necessary personnel to get Truck
Mounted Attenuator (TMA) lorry to be parked in front of the work zone if major roads
are involved i.e. road speed limit greater than >70km/hr.

c) Sufficient and suitable lightings for access & egress and work, whether natural or article
(minimum-50lux), shall be provided in every part of a workplace in which persons are
working or passing (minimum-200lux).

d) Provision of artificial light outside manhole and all portable hand-held lightings
provided in manhole must be fully insulated and shall be operated at a voltage not
exceeding alternating current (AC)50V between the conductor and earth or direct
current (DC)110V.

e) Supervisor to ensure night work PTW form is filled accordingly, Safety assessor to
endorse and Authorised Manager (AM) to issue the PTW before start night work.

f) Supervisor to allocate area for emergency assemble and all workers to be briefed on
escape route and nearest emergency assembly point prior to start work provide
emergency escape routes map if necessary and brief to the workers.

g) Ensure the Supervisor to check the site clean after work is completed.
