Sub: Application for lodging an FIR for my lost mobile phone

Respected Sir,

I, Suchismita Mukherjee Dasgupta, residing at Block CA 36, St no 252, Flat no 3B, Action
area 1, Newtown. Kolkata - 700157, lost my mobile phone yesterday while travelling from Sonarpur
railway station. I saw my phone last at around 3:40 PM when I received a call before boarding a train at
Sonarpur. After the call, I had kept the phone in my bag's pocket. Then, after boarding the train, when I
looked for the phone, I found it missing.

It was a Redmi phone (Model - Redmi 9 Activ, Colour – Carbon Black) with IMEI
numbers IMEI 1: 865946052856075 IMEI 2: 865946052856083. The phone was having a Jio sim with
phone number 7003990890. I hereby request you to kindly register an FIR for my lost mobile phone and
provide me with an attested copy of the complaint. I request you to kindly take necessary actions to
trace the phone and recover it.
