INFORMATION

The undersigned Assistant City Prosecutor hereby accuses Aku Sado of the crime of Homicide committed as follows:

That on or about August 12, 2013, Makati City within the jurisdiction of this court, the said accused, armed with a
bladed weapon, with intent to kill, did then and there willfully,unlawfully and feloniously attack, assault and stab one VIC
TIMA, thereby inflicting upon him a fatal wound which directly caused his death.

Contrary to law.

 PROMISSORY NOTE

FOR VALUE RECEIVED, I, JUAN DELA CRUZ, of legal age, single, Filipino, with residence
at Blk 2, Lot 1, Miraville Subd. Makati City, promise to pay without the need of demand to the order of
PEDRO JOSE, of legal age, Filipino, single, with residence at Unit 100, Tower 1 Dela Rosa, Makati
City, the principal amount of PESOS: ONE HUNDRED THOUSAND PESOS(Php100,000.00),
Philippine currency, within the period of one (1) year from the date of execution of this document,
with five (5%) percent interest per month, payable as follows:

a. The first payment of monthly interest shall be due on the first Monday of October 2020, and the
subsequent payment shall be due on the same day of the next following month.

b. Any payments made shall be first applied to the payment of the interest, and the remaining
amount, if any, shall be applied to the principal loan.

c. Any interest not paid shall be added on to the principal sum owing, and shall accrue interest as
principal from such time onwards.

d. It is hereby understood that should I default in the payment of interest for at least two (2)
consecutive months, the entire principal, interests, and other charges due on this Note shall
become immediately due and demandable without the need of demand.

e. In the event legal action is brought to recover the amount on the Note, I agree to pay the sum
equivalent to Twenty Five Percent (25%) of the principal and interest due and unpaid on the
Note as Attorney’s Fee.

f. All payments and performance of the obligations under this Note shall be made at the
residence address of Pedro Jose indicated above.
