Dust Emissions from a wood manufacturer

Root Cause - Management


Corrective Action – The management must place a responsible staff who will always be on site and
will enforce wood manufacturing to take place in an enclosed area with stack/chimney.
Effectiveness of Corrective Action – Dust emissions have reduced significantly since all rules of wood
manufacturing are being followed.
Preventive Action – Awareness must be created among the management about the harmful impacts
of Dust emissions on the living organisms and the environment. And strict warning to be issued by the
management on non-adherence of regulations.
Effectiveness of Preventive Action – The management have become more responsible upon knowing
the harmful impact of dust emissions and is taking extra precaution to prevent dust emissions.

II. Wastewater Disposal without Permit

Root Cause - Supervisor


Corrective Action – The Supervisor must immediately apply for a permit to dispose wastewater
Effectiveness of Corrective Action – The wastewater is now being disposed-off legally upon receiving
the permit from DM.
Preventive Action – Either the supervisor must be replaced by a more responsible person who knows
the importance of permits or else a warning must be issued to the supervisor along with a short
training on list of permits required for various activities within the company premises.
Effectiveness of Preventive Action – The supervisor is extra attentive with the permits and its expiry
dates, resulting in proper wastewater disposal without any trouble to the DM.

III. Land Contamination in a Diesel trading company

Root Cause - Incident/Accident


Corrective Action - Using one of the many techniques of preventing further spread of the Land
contamination by Diesel like excavating the contaminated soil and replacing it with clean soil, or
depositing immobilizing reagents into the land to stop further spread of the contamination.
Effectiveness of Corrective Action – The Land is free from contamination and the spread has been
prevented.
Preventive Action – The Company must invest in high quality tanks and pipes which will significantly
reduce such incidents. The company must also invest in hiring sufficient labor to conduct the tasks
efficiently with minimum incidents. Self-Monitoring inspections must be conducted and reported
such that these incidents are prevented before happening.
Effectiveness of Preventive Action – Incidents/Accidents have reduced significantly and many defects
are being detected during the monitoring inspections which could have led to several accidents.
