SUBJECT: Spot Report Re- Road Crash Incident (RIR in Physical Injury and Damage
to Property).

VICTIMS: 1. One (1) orange Yamaha Mio motorcycle with plate number 726 DGC
owned and driven by Maria Norine Calucin Serdeña, 58 years old (DOB April 20,
1964), married, housewife, resident of Brgy Baao, Mauban, Quezon, with NPDL D09-
10-007025 valid until 2024/04/20, without helmet.

SUSPECT: One (1) Isuzu elf with plate number KAM 9991 registered to Mr Gilbert M
Amante and driven by Rogelio Advincula Vinas, 54 years old (DOB May 8, 1968),
married, driver, resident of Green Meadows Subdivision, Red-V, Lucena City, with PDL
No. D09-94-049101 valid until 2023/05/08.

NARRATIVE: At about 2:00 PM of January 10, 2023, personnel of Mauban MPS

responded to a reported vehicular crash which transpired at Brgy San Isidro, Mauban,
Quezon. Investigation reveals that at about 1:30 PM of the same date, while victim was
driving an orange Yamaha Mio motorcycle with plate number 726 DGC at the Provincial
Road of Brgy San Isidro, this Municipality and while about to turn left with signal, she
was accidentally hit and bumped at the rear portion by an Isuzu elf with plate number
KAM 9991 driven by the suspect with the same direction. As a result, victim sustained
wounds and injuries in her body while both vehicles incurred undetermined amount of
damages due to said incident.

STATUS OF THE CASE: Amicably Settled.

2. This pertains to a reported road crash incident which transpired at Brgy San
Isidro, Mauban, Quezon on or about 1:30 PM of January 10, 2023 wherein the suspect
was identified as; Rogelio Advincula Vinas, 54 years old (DOB May 8, 1968), married,
driver, resident of Green Meadows Subdivision, Red-V, Lucena City, with PDL No. D09-
94-049101 valid until 2023/05/08.

3. Please be informed that at about 2:00 PM of January 10, 2023, personnel of
