BORROWER REQUEST FORM

TERMS AND CONDITIONS IN BORROWING ICT EQUIPMENT

1. The equipment shall be returned before or on the approved drop off time.

2. The borrower is liable for any damage or loss of equipment. If the equipment is
damaged or loss, it must be paid or replaced according to the value determined by
the School Property Custodian.

3. For equipment that are still repairable and not under warranty, the borrower will
shoulder all the expenses for repair including labor and replacement parts.

I hereby agree to the terms and conditions stated in the policies in borrowing
ICT equipment of Citrus National High School.
