1) I confirm that my ward / son / daughter is physically and medically fit to undertake the rigorous
sports activity.

2) I confirm that my son/daughter will adhere strictly to the rules and discipline of the game.

3) In case of any injury, accident, or sickness, while playing the sport, I or any member of my
family shall not hold the School or the coaches wholly or partially in any way.

4) I confirm that once my child is selected for the game, he/she will play for the period of the whole
academic year and not change it .

5) I am aware during competitions my child will need to do extra practice which may happen
during school hours by missing some academic sessions or before or after school hours, as may
be necessary.

6) I am aware that if my child is not regular for practice or misbehaves in any way or causes any
kind of damage to school property, he/she will not be allowed to participate in competitive events
and will be disqualified from sports team. The same will be applicable in case of indiscipline at
any point of time. The school and coach decision in this case will be final.

7) I am aware that for certain events he may need to buy a different uniform for competitive events
at an additional cost.

8) I am aware that a minimum of 60% in academics must be maintained by my child in terms of

his academic records.
