Guide to Creation of Inspection Reports in Meridium/APM

Step 1: Log into Meridium - http://dvep.cfidv.cfiorg.net/meridium/


o Use the same log in credentials as computer login information.
Step 2: Click on the Integrity (+) tab on the left toolbar. Proceed to click on Inspection
Management.

Step 3: Click on the filter icon shown by Inspection Management Overview.


o You will then filter by CFI/Plant/Equipment or circuit ID.
o Make sure to click on the second option.
Step 4: Create the Inspection Report.
o Click on the + on the far right to create an inspection report – Create Inspection.
o Select the Inspection Event/Checklist and Inspection Tasks that apply. Click Create.
*Tip: When creating an inspection report, make sure to select the correct inspection event and inspection tasks that
apply. This information will come from the temporary inspectors during the Turnaround. They will save the report
as a Word document & you will transfer the information from that file to Meridium.

Step 5: Copy all information from the word template supplied to you by Eric and the temporary
inspectors into Meridium. This includes:
o Inspection Summary
o Reviewers Name
o Final Approver Name
o Checklist Findings (ALL have to be filled out – nothing should be left blank)
 Please note that the checklists are not in the same order in Meridium as they are
on the Word document – we are currently working to resolve this issue.
o Inspection Status (should be changed to PENDING approval once report is completed)
o Reference Documents should be added if necessary.
*Tip: Every time you change/add something in Meridium, press save on the right hand side. It will become your
best friend. Trust me. 

Step 6: Editing Reference Document for Importing:


o There should be the following information to combine as a reference document to add
into Meridium: (ALL of these documents need to be converted to PDF then combined in
the following order:)
o P&IDs
o GA Dwgs
o Any UT/PT reports done during the Turnaround
o U1As, U2s, etc.
o Inspection Picture Template
*Please note: there are instances when all of this information will not be in the folder – Sometimes there
aren’t any extra reports needed (UT/PT/MTs, etc), no Gas found, and even no P&IDs found – This is typically
only for Medium criticality equipment.
o Reference document should be named INSP-XXXX_Inspection_Report_Reference_Doc
Step 6: Add Reference Document to SharePoint/Drop-off Library:
o Follow this link: http://dv-intranet.cfidv.cfiorg.net/documentcenter/eng/DropOffLibrary/Forms/AllItems.aspx
o From here, you should be able to drag and drop your reference document
o Once the document has been uploaded, click on the 3 horizontal dots to edit the
properties:
o The edits should mimic the below image. Make sure that the Operating Unit,
Equipment, Equipment ID, Inspection Report ID, & Inspection Year are correct.

o When the edits are complete, press Submit and it will bring you to the final
location. Copy the shortcut on the screen.

Step 7: Add Reference Document to Meridium:

o On the Inspection Data tab/Inspection Details/ there is a symbol to the right.


Click on it and and select 0 Reference documents
o Click on the + button to add a document. Please make sure when editing the
fields, the ID and Description look like this:

o Add in the copied shortcut from SharePoint to the Document Path field.
o Make sure to SAVE before closing.

Step 8: Go back to the Inspection Data/Inspection Details Checklist page & change the
Inspection Status from Draft to PENDING APPROVAL and press Save.
o This will notify Eric that the inspection report is ready for review in his inbox.

Procedure for uploading documents into SharePoint

1. Follow link to “Drop-Off” page in SharePoint:


1.1. http://dv-intranet.cfidv.cfiorg.net/eng/DropOffLibrary/Forms/AllItems.aspx
2. Drag and drop the file you are trying to upload into the drop area.

3. Click the button with the 3 horizontal dots, then on edit properties.

4. Fill in the required information as follows (see example below):


4.1. Choose content type. We will only be using report and recommendation reference documents
for this TA.
4.1.1. Inspection Report Reference Document
4.1.2. Inspection Recommendation Reference Document
4.1.3. Inspection Task Reference Document
4.2. Enter the document name, similar to the one shown in the example below
4.3. Enter the document title, similar to the one shown in the example below
4.4. Choose the operating unit from pick list or typing it in the field.
4.5. Choose Equipment or Piping
4.6. Enter the Equipment ID with no spaces or “-“. Ex. 101E, 102CA, TK510, etc.
4.7. Enter the Meridium inspection report ID. Ex. INSP-123
4.8. Choose the year
4.9. For the Recommendation content type, you must also enter the “Meridium Recommendation
ID.” Ex. REC-120
5. Once the all the required information is entered, click the save button.

6. Once saved, the screen below will pop up showing the link to the file location.

7. Copy this link and paste it into the “Document Path” for the reference document in Meridium. See
below

This documents the configuration items for CF Donaldsville. It will cover the changes and additions to the
baseline Asset Performance Management (APM) software baseline product. Baseline functionality is covered by
the application Help. This for V4.3.0.4.0 version of APM.

3.1 Assets
CFI CMMS system is Avantis. A configuration of APMConnect has been implemented to bring Functional
Locations (Flocs) and Equipment in to APM. These are built out in the Asset Hierarchy.

Users in the ‘CFI Asset Data Manager’ role have the ability to populate the following fields on Equipment, data
specifically managed in APM.

Note that Assets originating from Avantis have CMMS value of ‘DVILLE’. New Assets cannot be created directly in
APM for CFI. They can be added via a data loader. Any asset added this way will have a CMMS value of APM.
Care should be taken on giving the ability to use data loaders to a user. Using a data loader effectively make a
user a ‘superuser’ in respect to data.

CFI Criticality Rating is a picklist. The value displayed in brackets e.g. (32050) is the actual code stored in the field
and precedent the displayed description. He codes match Avantis values.
3.1.1 Piping Circuits
These do not originate from Avantis but can be added via a data loader. They have a CMMS System value of
APM. The functional location to which they are attached do come from Avantis.
3.2 Taxonomy
Equipment are classified by populating the Object Type on the equipment from the classification in Avantis.
These are mapped to standard GE taxonomy via the Taxonomy Mapping family.

The taxonomy can be reviewed in the Taxonomy Category, Class and Type families.

The taxonomy as applied to each equipment can be seen on the datasheet (See Figure 7 Equipment). Note that
the value mapped form Avantis is in the ‘Class’ field. This is actually the Class Description. The class value field is
not displayed on the Equipment datasheet.

3.3 Work History and Work History Detail
Data that comes in from Avantis. Work History is combination or Work Requests and Work Orders. A key point
for the CFI implementation is that only Avantis work that initiated by either recommendations or inspection
tasks will be brought back in to APM. Work History Detail are specific actions / tasks / operations / steps that
panned for each work order. Only actions of certain types that indicate the disposition of the work with respect
to inspections or recommendations will be brought in to APM. The purpose is to indicate the readiness of
equipment to the inspection group for the inspection action, and similarly so inspection can communicate back
through the process to Avantis the inspection step has been completed. Sometimes this can be a multistep
where an inspection is in progress that uncovers a condition that requires redress by maintenance during the
inspection cycle. Since all the data comes from Avantis all fields are read only i.e. disabled.

3.4 General Recommendations
These are created directly from the asset. They are not associated with any specific APM work flow. However,
for CFI they are configured the same as for inspection recommendations so see Inspection Recommendations.
The only difference is the headline has no option for Temporary or Permanent repairs.
3.5 Initial Dashboard
When a user is initially added to APM they are allocated whatever has been set up as the default. This is found
under Admin\Security Manager\Users Defaults

A CFI Master dashboard has been created for CFI. Primary function is a starting place to other dashboards or
object. CFI can add to this for other global site content.
Note: that a user can change what they want to use as their initial dashboard at any time.

The initial content provided is a link to a dashboard for Operators.

General note. The overview dashboard and queries that underly the product tiles can be modified at any time.
The following indicates the content as deliver to CFI by GE at go-live.
4.1 Inspection Management Overview
4.1.1 Inspection Management Overview Dashboard
The product dashboard has been update to show CFI specific content.
This can be found in the catalogue at:

It has 4 key widgets to indicate where the inspections are in the work process. As can be deduced from the
titles, each widget presents data in context of the current user. A key point is the data displayed is in context of
where the user is in the CFI Asset hierarchy. If they are at the top level of ‘CFI’ then it will be all assets. If they are
at a process unit level such as AM3 then it will data for all assets below that level
4.1.1.1 YOUR ASSIGNED TASKS WAITING FOR DOCUMENTATION (INSPECTOR)
Public\Client\Inspection\Queries\Task Waiting Inspectors Review – Hierarchy
Shows the current user, an Inspector, when they have been assigned tasks to complete the necessary supporting
reference document package for the analyst to review. When they have completed adding the reference
documents they will check the ‘Ready for Analysts Review’ on the task datasheet.
4.1.1.2 YOUR TASKS WAITING FOR REVIEW (ANALYST)
Public\Client\Inspection\Queries\Reliability Analysts Task Review – Hierarchy
Shows the current user when tasks that have had the reference documentation package completed are ready
for their review.
Each equipment belongs to a Functional Location that is in process unit. Each process unit is assigned a
Reliability Analyst (see Figure 6 FLoc). So, the query retrieves the analyst for the Unit the equipment is in to
match against the current user. The task will appear until the work request is raised.

4.1.1.4 YOUR RECOMMENDATIONS

Lists the inspection recommendations where the current user is the current state owner for that state; Pending
Review or Pending Approval or Approved. Once the state is either Rejected. Superseded or In-Progress or
Completed, and the current user in one pf the previous state owners it will appear on the list for 14 days after
that state has been selected.
If the recommendation requires inspection follow up and the recommendation generated a Work Request, it
also looks at the operations in the work history detail to see when Maintenance say the recommendation is
ready for inspection.

This 5th widget comprises of links that navigate the user to further information.

This the same dash as the overview dashboard but no asset hierarchy is applied so all relevant data is
always provided. It will be CFI’s decision if they wish to maintain this mirror dashboard. If so, any
changes made to the overview dashboard will need to be correspondingly made to this dashboard.

4.2 Inspection Tasks


Inspection Task plan the inspection work. For CFI state management has been implemented, specifically to
request changes to the task. Once a task is set up only users in the role of ‘CFI Task Administrator’ can modify
specific task fields. All task are created in a ‘Scheduled’ state. Tasks that are no longer needed are marked as
‘Completed’.
For CFI task go through a couple of internal phases. When a task is coming due, the contract supervisor will
assign the task to an inspector. The inspector will then plan the inspection work by attaching all relevant
documents as reference documents. When that is ready the inspector will check the ‘Ready for Analyst Review’.
Once reviewed the analyst can check the ‘Create Work Request Now’ flag to create the WR in Avantis.

If a change to the task is needed the inspector completes the ‘Reason for Task Change Request’ field and then
changes the state to ‘Request Change’

The Analyst then sets the task back to Scheduled having made the requested change.

4.3 Inspection Events
These are also referred to as the documents or reports where the results of the inspection are recorded.
CFI can use any of the product supplied but will predominantly use Checklist and General Inspections.

Note: all checklist events are the same. What differs is the content for the checklist findings on the left. CFI are
using the baseline product findings content. These can found in the Checklist Categories
(MI_CHECKLIST_CATEGORIES) and Checklist Finding Items (MI_CHECKLIST_FINDING_ITEMS) system code tables,
accessible by an administrator only.

The basic functionality of inspection events is essentially the same as product with respect to the review and
approval process. CFI have an additional intermediate review of the inspection before the final approval by the
designated CFI individual. The integrity of the report is then same as product where the inspector locks the
report from changes by others, but the final lock to ensure not changes are made after final review is done by
the final approver. For CFI it is the Final Approver who is task with the doing the Inspection Confidence
Evaluation which later feeds back to the Risk Based Inspection RBI module.
4.3.1 Specific event fields
The following are fields added to the event datasheet specifically for CFI

Fit-For-Service Opens the Fit-for-Service datasheet in Record Explorer
Equipment Serial No. Passed from corresponding field for the Equipment being inspected.
NBIC Number Passed from corresponding field for the Equipment being inspected
Replacement Count Passed from corresponding field for the Equipment being inspected
Work Order Number Populated by the Work Order in the Work History family for the WR
generated by the task being addressed
Inspection Status Product filed by values have been added system code table MI_STATUS for
CFI. These are
RE-REFERRED ( Re-Referred to Report Owner )
FINAL APPROVAL ( Final Approval )
Inspection Report Owner Product field but CFI derives this list from the ‘CFI Inspector’ security role
Reviewers Name Product field but CFI derives this list from the ‘CFI Inspection Reviewer’
security role. For Donaldsville this is typical the contract supervisor for the
inspectors.
Final Approver Name CFI field. List derived of users in the ‘CFI Inspection Approver’ security role.
Two addition fields that the user does not interact with are also included to
ensure the Final Approver functionality is the same as the product fields.
These are
• Final Approver User ID
• Final Approver Key
Update Task This updates the task selected in the Tasks Addressed fields. Key points are
that the inspection completion date is is passed to the Task Last Date.
The Reference Documents added to the task to support this inspection are
removed.
The ‘Ready for Analyst Review’ flag is cleared is not already cleared.
Work Request Number / Order Number fields on the task are cleared ready
for the next plan cycle.

The recommendation content is an adaption of the baseline content with specific items configured for CFI as
follows

FIELD FUNCTIONALITY
Recommendation Headline Product field but user can manually type a headline or can
select either Temporary or Permeant Repair.

EWR/MOC Tracking Number CFI field where user can enter the track ID.
This is numeric value for CFI.
EWR/MOC Tracking Status CFI free format field
Recommendation Priority Product field but picklist modified to match Avantis priorities
as values passed to Avantis if WR needed.
From system code table ‘MI_PRIORITY’
Work Reference Type Picklist from Work Request Type – Recommendation
(CC_WR_TYPE_REC) system code table.

Follow-up Inspection Required? Required CFI field that indicates if the work requested needs
a follow up inspection. Value is passed to Avantis so Mtce
know they need a Hold point.
Work Request Required CFI field Flag that indicates if the recommendation will need a
work request. When checked and the recommendation
approved, the ‘Work Request Created Flag’ will be checked.
Work Request Created Flag Product field adapted for CFI. Disabled for user. System
checks this when recommendation is approved and ‘Work
Request Required’ is checked.
Work Request Reference Product field that gets the ID of the Work Request when
created in Avantis.

4.5 Fitness for Service
CFI specific family. Initiated from link on inspection event datasheet. Used as a sign-off form to approve
equipment’s return to service. The printable (SSRS Report) version of this is also included in the printable
inspection reports.

FIELDS FUNCTIONALITY
Description of Work Populated on creation with each of the task ID for the tasks
being addressed on the inspection event. User can then edit
and add to content of the field as desired.
Ready For Authorization Flag to tell the Reliability Analyst that there is a FFS to
approve.
Once checked, deletion of the FFS is not allowed.
Note: it is not a requirement that this field be checked. It
used as means of alerting the analyst there a FFS for their
attention.
Equipment(s) Populated by the system with the equipment the FFS is for.
User cannot change
Area / Location Populated by the system with the area / location the
equipment is in. User cannot change
Fit-for-Service? Flag that the only a Reliability Analyst can check to approve
the FFS. Note: it does not have to be the RA assigned to that
Unit. Also it does not require that the ‘Ready For
Authorization’ has been checked. The RA can do this at any
time.
Once checked only that user can uncheck it. No other user
who is a RA can uncheck it.
If it unchecked all the fields that were disabled by this flag
become enabled and the authorization name date fields are
cleared.
Once checked, deletion of the FFS is not allowed. All fields are
also disabled.
Inspection Reference Populated by the system with inspection that determined the
condition of the equipment that the FFS is generated from.
Note: the FFS is related back to the inspection through the
‘Has Fit-for-Service Authorization’ relationship.
Approver ID Stores the ID of the analyst checking the FFS box. Displays the
ID and user name. Populated by system. User cannot edit
Authorization Date Stores date and time the FFS box was checked. Populated by
system. User cannot edit
Attachments Free format field to document any attachments that will be
sent with the FFS
Comments Free format field to add any comments
Work Request Number Populated by system from each of the tasks shown in the
Description of Work. Can be populated manually by the user.
Work Order Number Populated by system from the Work History family for the
Work Request Number above. Can be populated manually by
the user.
