    Thank you for the information you provided regarding the price list and what types of products you offer
    to our company. We are pleased to order health service at Hermina Hospital for our company, with the
    following types of health services:

    1. Medical Check Up
    2. Covid-19 Vaccination Program, Sinovac Type (Dose 1 and Dose 2)
    3. Antigen Swab Test
    4. Test PCR (Polymerase Chain Reaction)

    Based on the negotiations that we have done and we have agreed to, the payment is made after all of
    our company's employees have taken a medical test. Payment is sent through BCA Bank on behalf of
    “Rumah Sakit Hermina” with a discount of 20%.
