Valid and Current Mayor’s Permit; b) PHILGEPS Registration No.; c) Latest Income/Business Tax
Return, and c) Omnibus Sworn Statement
4. Quotations should be valid for 15 days counted from the deadline of submission.
5. Delivery Period: Within 15 Calendar Days Upon Receipt of PO
