AFFIDAVIT OF CONSOLIDATION

I, MARIA ELENA M. ECHALUSE, of legal age, married, Filipino citizen and

resident of Brgy. Uacon, Candelaria, Zambales, after having been duly sworn to in
accordance with the law, depose and state:

1. That I am the President of the BANKO ZAMBALEÑO, INC. (BZI) (A

RURAL BANK) and authorized by its Board of Directors to execute this
affidavit for and in behalf of the bank;

2. That on 21 September 2018 the Sheriff of the Province of Zambales, on

verified request of our bank, conducted a public auction sale of the following
described real properties;

MORTRGAGOR - DINA T. ECLEVIA

TCT. NO.- 044-2015001858 and Tax Declaration No.003-0406

Location : Brgy. Catol, Candelaria, Zambales
Boundaries : Line 1-2 South, Lot 1310 Candelaria
Cadastre, Line 2-3 West, Lot 1321-
D, PSD-03- 119530, 3-4 North, Lot
1321- F,PSD-03-119530,Line 4-1 East,
Lot 1321-H, PSD-03-119530

As a consequence of which he executed on 25 September 2018 a

Certificate of Sale in favor of the BANKO ZAMBALEÑO, INC. (BZI) (A
RURAL BANK) as the bidder for FIVE HUNDRED THIRTY-THREE
THOUSAND TWO HUNDRED FIFTY-THREE 32/100 ONLY (P530,253.82)
PESOS, which on 23 February 2018, was annotated at the back of the of the Tax
Declaration of above mentioned property;

3. That the period of redemption of therein mortgagor expired on 26 September

2019, and up to the present, the mortgagor or any person authorized by them
have not redeemed the property described;

4. That this affidavit is executed for the purposes of Consolidation of title over
the above described property, in favor of the BANKO ZAMBALEÑO, INC.
(BZI) (A RURAL BANK) and the issuance of title to its name by the Registry
of Deeds of the Province of Zambales;
