SUBJECT : EXPLANATION

With reference to the NTE from your office dated 27 November 2019 which was received by the
undersigned last week, I would like to seek for your consideration for not wearing my uniform
last November 26, 2019 while conducting visitation/inspection at FLI Verde Spatial.

Our president actually noticed the incident where he directed me to wear my uniform that was
complied with upon receipt of instruction. On that particular date I have with me my official
uniform so it was not intentional to deviate with our policy governing office uniform.

That I will make it sure to always wear my official uniform in the future.
