Dear Mr Alan Gabriel Prieto

We are Grade 11 STEM students from Valenzuela City School of Mathematics and
Science. Our group are currently looking for information towards the career path in the
Engineering Field. One of our members, Dustin Ong suggested that you are a qualified and
excellent source of information.
Our group would like to request an appointment with you on Sunday, January 15 for
some of your time. It takes approximately 30 minutes to an hour, for our project in Personal
Development. The topics will include your experiences in your studies and experiences in the
field of Engineering.
The interview will consist of 7 questions, and if given permission some follow-up
questions or a motivational speech.

Interview Questions:
1. Why did you choose Engineering Field?
2. What is/are the biggest challenges you experienced?
3. How were you able to overcome the challenges?
4. How will you prepare for the Board Exam or other licensed Exams?
5. So far, what’s the best experience related to Engineering Field?
6. Where would you like to be in your career 5 years from now?
7. What do you think are the skills most essential for aspiring Engineers?

We would also like to request a video recording of the interview along with your permission to
make a transcript. There are other options as well should you have any concerns for you privacy
or schedules.

The data gathered will be presented to our class. If possible share your personal
information, we would retain the info and share it with the class. However, if we’re not given
permission to share, your personal information will be omitted. All personal information shared
for this project will only be viewed by the members of our group, our classmates, and our subject
teacher. If you have any inquiries, feel free to contact us through messenger, email

Consent for participation in an interview


1. I agree to participate in a performance task led by the students from Valenzuela City
School of Mathematics and Science. The purpose of this document is to specify the
terms of my participation in the project through being interviewed.
2. I have been given sufficient information about this performance task. The purpose of my
participation as an interviewee in this project has been explained to me and is clear.
3. My participation as an interviewee in this project is voluntary. There is no explicit or
implicit coercion whatsoever to participate.
4. Participation involves being interviewed by the students from Valenzuela City School of
Mathematics and Science. I allow the students to take written notes during the interview.
I also may allow the recording (by audio/video tape) of the interview. It is clear to me that
in case I do not want the interview to be taped I am at any point of time fully entitled to
withdraw from participation.
5. I have the right not to answer any of the questions. If I feel uncomfortable in any way
during the interview session, I have the right to withdraw from the interview.
6. I have read and understood the points and statements of this form. I have had all my
questions answered to my satisfaction, and I voluntarily agree to participate in this study.
