AFFIDAVIT

I, Mrs. Gemma Veronica Lopes aged years, Indian Inhabitant residing at

do hereby state on oath as follows:-

1. I say that I was born on .

2. I say that my maiden name was Gemma Veronica Braganza and

after my marriage my name is Gemma Veronica Lopes.
3. I say that my middle name is “ Veronica “.
4. I say that in my School Leaving Certificate, Passport and Aadhar Card

my middle name is mentioned as “ Veronica “.

5. I say that in the Birth Certificate issued to me by the Municipal

Corporation of Greater Mumbai my middle name, “ Veronica “ is not

mentioned and I desire that the said name be included therein.

6. I am making this affidavit to enable the concerned Authorities to include

my middle name, “ Veronica” in my Birth Certificate and to make the necessary

changes in the official records pertaining to my middle name.
