Notes:
1. Deduction of variance from the overall rating is to remove the biasness from the KPI
rating as appraisers can be generous on KPI rating but very reserved on Competency
rating.
2. Only the 1st page of this form must be distributed among the staff.
3. This form could be used provided the KPIs and Competencies are agreed upon between
the appraiser and appraisee.
4. HR Department’s page must not be exposed to the staff.
5. Final Decision must be taken by the Board of Management.
