Filipino and a resident of Gran Plains Subd., Tabuk Suba, Jaro, Iloilo City, Philippines, and
EDDIE N. MANCILLA, of legal age, married to MA. ELISA P. MANCILLA, Filipino and a
resident of Brgy. Poblacion, Janiuay, Iloilo, Philippines, having been duly sworn to in
accordance with law, do hereby depose and state:

1. That we are the only surviving and lawful heirs of CONCEPCION MANCILLA and
VICTOR MANCILLA, SR.;

2. That we are married to the above-named Spouses, namely:

EDDIE N. MANCILLA, married to MA. ELISA P. MANCILLA;

3. That we are executing this affidavit to certify or attest to the truth of the foregoing facts
