dependable and reliable without sacrificing our vision, mission, and core values. You were chosen
because we see in you the character, knowledge, and skills to be good leaders. Keep the light
burning. This will be the basis of your performance assessment. Thank you.

Guidelines:
1. Check the appropriate rating. Please rate honestly and objectively.

Please be mindful that the assessment will be made in the following categories:

Note: Applicable to all evaluation tools if deemed necessary: Grades 1 and 2 are considered
failures. A rating of 3 to 5 is promoted. Once the employee or staff failed twice in any of the
categories, every failed rating will be equivalent to 2-point deductions to the overall score. The overall
score will be used to determine the next best employee of the month, the employees’ standing, and
the basis for re-appointment of the designated position. An assessment exam is also given to all
teaching and office employees. The passing overall score is 75%. In case of a tie, an exam will be
given to determine the leading employee.

1. Leaders should reply to the inquiries or comments of their head/s within 24 hours. Unable
to reply will incur a 3-point deduction / unattended inquiries to the performance overall score.
2. Passing of any documents, emails, or any request within 48 hours should be done. Unable
to do so will incur a 5-point deduction / late day on the overall performance score.
3. Unable to do the requested tasks within 48 hours will incur a 5-point deduction for every
late counted day.
4. In case of a personal or family emergency, the assistant head/s should inform the head/s
on the dot (Immediately) Medical certificate.
5. Any unreported incident/s will incur a 5-point deduction, but if the incident unreported has
caused heavy damage (after an evaluation) to the reputation of the school or anyone
connected to it will have serious implications.
6. Submission of all papers works requested should be submitted on the 3 rd working day from
the issuance of the request. Unable to do so will incur a 5-point deduction for every late
counted day.
7. Submission of the requested documents or doing the requested task within 48 hours (with
no corrections) will incur 2 bonus points. If the corrections are made the resubmission of the
paper work or tasks accomplishment will be counted from the day the correction was made.
(3rd day rule will be applied)
