Re: Mr Bob Dawson, DOB: 25th September 1924

I am writing this letter on behalf of Mrs Elizabeth Dawson to request a home visit
for her husband Mr Dawson who has impaired mobility. Your expert advice and
assistance to improve his mobility will be greatly appreciated.

Initially, on 18th May 2008, Mr Dawson first visited our facility with a complaint
of grazed left knee due to fall from stairs. Daily visit was arranged for wound
dressing and help with showering according to the request of GP.

During his subsequent visit on 19th May 2008, Mr Dawson's knee was redressed
and there is no sign of infection. He reported normal aches and pain; however, he
was able to mobilize with the help of his wife.

On today's review, Mr Dawson's knee condition has improved. Although he is


advised to use a walker or walking stick to improve his mobility, his wife said Mr
Dawson never learned the proper use of stick. Therefore, Mrs Dawson requested
a home visit from you to assess her husband.

Mr Dawson is married and lives with his wife. He had a cerebrovascular accident
four years ago as a result, he has slight speech impairment alone with left sided
residual weakness and slightly balance impairment.
On view of the above, it would be greatly appreciated if you could visit Mr
Dawson and educate the use of stick.

Correction

I am writing this on behalf of Mrs. Elizabeth Dau, wife of Mr. Bob Dawson, who
is recovering from a left knee injury. He requires regular physiotherapy home
visits rehabilitation therapy to improve his mobility.

Mr. Dawson presented on 18 May 2008 with a badly grazed knee following a fell
at home. He was advised for daily assistance in showing and dressing by his
General Physician. Currently he is able to walk a short distance with the help of
his wife, and his wound has no infection. The doctor has advised him to use
walking stick in order to improve mobility; however, is unskilled in using it.

Mr. Dawson lives with his wife. He has a history of cerebovascular accident
which occurred four years ago. Although he has recovered successfully, he still
has left sided weakness, slightly slurred speech and impaired imbalance.
Therefore, he walks with a limp.

In view of the above, it would be appreciated if you could visit Mr. Dawson and
assess his mobility pattern. Please educate him regarding the proper usage of
walker. In addition to this, kindly provide assistance and support to improve his
mobility.
