Dear Nurse

Re: Mrs. Mavis Brampton, Aged: 72 years

This letter is being written to refer Mrs. Brampton, who has been diagnosed with pleurisy. She
requires regular observation and ongoing care following her discharge on 15th January.

Initially, on 10th January 2011, Mrs. Brampton first admitted to hospital with complaints of
fever and breathlessness. Upon assessment, she had elevated blood pressure and temperature.
During hospitalization, a chest X-ray was performed which confirmed the diagnosis. She was
treated with intravenous Amoxicillin and oxygen supplementation.

Upon her follow-up on 12th January 2011, Mrs. Brampton had productive cough for which
sputum culture was done. She was commenced on Pravastin 20mg and Celecoxib 100mg daily
and recommended to take a low-dairy diet.

On today's review, Mrs. Brampton advised to start breathing exercise in order to keep a non
smoking regime. She is currently on Nicotine patches and Zyban 150mg twice daily.

Mrs. Brampton is a widow and an active member of community. She has been smoking since
the age of 18 and smokes 10 cigarettes a day.

In view of the above, it would be highly appreciated if you could monitor Mrs. Brampton's
medication compliance and side effects of Zyban. Kindly, arrange a assistance for nutritious
meal, housekeeping and shopping. In addition to this, encourage her to continue Nicotine
patches until the control of smoking cessation.
