DEED OF REAL ESTATE MORTGAGE

KNOW ALL MEN BY THESE PRESENTS:

WE, _____________________________, (hereinafter, “Mortgagor”) Filipino Citizen, of

legal ages, and with residence address at ____________________________, for and in
consideration of a loan in the sum of ___________________________ Pesos
(Php____________.00), Philippine currency, to me and in hand paid by
______________________, (hereinafter, “Mortgagee”) Filipino, of legal age, married, and with
residence address at Atimonan, Quezon, do hereby convey, by way of MORTGAGE, unto the
said ____________________, his heirs and assigns, that certain parcel of land, together with all
the buildings and improvements thereon, situated in ____________________, particularly
described as follows:

of which real property we are the registered owner in accordance with the provisions of the Land
Registration Act, my title thereto being evidenced by Transfer Certificate of Title No.
______________ of land Registry of Lucena City: PROVIDED, HOWEVER, that if we, the
mortgagor, shall pay or cause to be paid to said mortgagee, his heirs or assigns, the said sum
of _________________________ Pesos (Php_____________________.00), within the period
of Two (2) years from and after the execution of this Mortgage together with the interest thereon
at the rate of thirty six per centum (36%) per annum, then this Mortgage shall be discharged
and of no effect; OTHERWISE, failure on my part to pay my obligations to the mortgagee, the
balance thereon shall immediately become due and payable without demand or notice and the
mortgagee may foreclose on the said real estate mortgage executed to secure the same.

For purposes of foreclosure, the mortgagee has been appointed effective upon default, as my
attorney-in-fact, and I hereby agree that the said mortgagee may enforce his rights herein
without judicial proceedings by causing the above-described property to be sold at public
auction, in accordance with Act No. 3135, as amended by Act No. 4118.
