ACKNOWLEDGMENT
This instrument consisting of three (3) pages, including the page where this
acknowledgment is written, refers to a Deed of Absolute Sale of a parcel of land
including all improvements found therein identified and known as Lot 1255-B with an
area of 1,845 Sq. Meters, more or less, situated in Jaro, Iloilo City, and covered by TCT
No. T-T-113152 signed by the parties and their instrumental witnesses on each and every
page thereof and sealed with my notarial seal.
This Deed of Absolute Sale is made, executed and entered into by and between:
GAUDIOSO P. GEDUSPAN II, of legal age, married, Filipino and a resident of B5 L17
PRJ, Parc Regency, Brgy. Ungka II, Pavia, Iloilo, Philippines, hereinafter referred to as the
BUYER; and
WITNESSETH:
WHEREAS, the herein SELLER is the true, exclusive, absolute and registered owner of
one (1) unit firearm, particularly described hereunder, as follows:
FOR THE SAME CONSIDERATION, the herein SELLER also waives and/or relinguish
all rights, interests and participation to the BUYER who is a also a valid holder of a License to
Own and Possess Firearm (LTOPF) with I.D. No. 2019I-GPG0653247-RCSU6 valid until
September 7, 2023.
IN WITNESS WHEREOF, we have hereunto set our hands this _________, 2020, Iloilo

their respective valid identification documents the numbers of which are reflected below their

I am the sole surviving heir of the late Jan V. Baltazar, who died intestate and without
will or debts to be paid on September 4, 2017 in Iloilo City;
During his lifetime, he is the true, lawful and registered owner of one (1) unit motor
vehicle particularly described as follows, to wit:
That pursuant to Section 1, Rule 74 of the Revised Rules of Court of the Philippines, I
hereby adjudicate unto myself the above-described motor vehicle free from all liens and
encumbrances.

thereof and that the sale for the consideration of ONE MILLION FIVE HUNDRED
THOUSAND PESOS (P1,500,000.00) includes the existing structures and
improvements standing thereon.

their respective identification cards the numbers of which are reflected below their names
and signatures, known to me and to me known to be the same persons who executed the
foregoing and that the same are their own free and voluntary acts and deed.
This instrument consisting of Two (2) pages refers to a Deed of Adjudication with
Waiver of Rights of Assessor’s Lot No. 276 situated in Granada St., Poblacion, Guimbal,
Iloilo, with an area of 0.0852 Hectares, more or less, signed by the parties and their
instrumental witnesses on each and every page thereof and sealed with my notarial seal.
Sir:
I am the true and lawful owner of one (1) unit TOYOTA RAV4 2014 with Plate No. AAL-
8354, Serial/Chassis No. JTMYF9EV90J011471 and Engine No. 2AR-E846475.
The registered owner is my cousin. I purchased this motor vehicle for my mother the late
Nelida Uy-Dellota in 2017 when she became sickly so that she can use the same for her
hospital visits. After the sale, and because my mother will be the one to use the same,
he delivered the original Certificate of Registration (CR) and Official Receipt (OR) as well
as the Cancellation of Mortgage from BDO to her. My mother died on March 14, 2019.
After her demise, we started using this motor vehicle until my son Neil Warren Uy
Dellota was flagged down May 24, 2019 by the elements of your police unit and this MY
unit impounded. It was then that we discovered that the original CR/OR, the original
cancellation of mortgage earlier given by my cousin Derrick Uy Chua and the previous
copy of the Deed of Sale he issued to me were all misplaced due to the fact that my
mother went in and out of the hospital for one year before she finally succumbed to her
illness.
There was never an intention to conceal anything by reason of the sale of this motor
vehicle to us by our cousin Derrick Chua. In fact, we were able to check with the Land
Transportation Office (LTO) and verified the vehicle history of this TOYOTA RAV4 2014
with Plate No. AAL-8354 and the same does not bear any alarm and that it showed that
it was purchased from a legitimate source having also shown the Vehicle Sales Invoice
for the said MV unit.
We fervently request therefore that you may be kind enough to release the same MV
unit from your custody so that we can properly reconstitute and straighten once and for
all the documents covering the herein motor vehicle.
As a sign of good faith, the registered owner Derrick Chua executed yet another Deed of
Absolute Sale in my favor if only to confirm the legitimacy of the sale. We will also be
transferring the ownership of this MV unit as soon as the relevant documents are put into
place.
This is of course with the undertaking that we will always make the same unit available
in case you need it for further investigation.

German M. Lopez, and subsequently by virtue of a Waiver of Rights covering the same
subject property dated 17 November 2007 in favor of Jose Soriano Colmenares per Doc.
