    Instructions in filling out the form.

    1. Secondary school ICT Coordinators and Elementary District ICT Coordinators will go to this link:
    https://forms.gle/epqtctFEw4mPmkTc8
    2. Each secondary school ICT coordinator must create an excel file with this template. Use this
    format of filename before uploading the file. <name of school_school id no.>

    3. The District ICT Coordinator in the Elementary shall create a workbook (excel file) of all the
    participants in the district by school using the template below. Use this format of filename
    before uploading the file <Name of District_Congressional District>

    Note: Only Secondary school ICT coordinator and Elementary District ICT Coordinator are
    allowed to upload the file of participants.
