Supervision Discussion:

- Discussed with Yvette regarding the Yvette about the incident that happed on 01.01.2023. MS was
left in the commode and was found on the floor for unknown duration.
- Yvette’s recount evidence that she did not know that MS was not supposed to be left alone in his
room while he is using the commode. That there were some miscommunications amongst members
of staffs on that shift.
- She felt that she does not want to blame anyone about the whole incident as it does not seem to be
right. She admits that she is equally accountable to the situation.
- She said that she does not want any conflict with other care staff as she is new in the unit but she
has observed that some care staff takes so much time in taking their breaks. She has also
mentioned that care staff should not take their breaks by two. She thinks that having two people go
on breaks together will leave the floor with one care staff which would be difficult to manage.
- She felt that day was very busy, that having 4 care staff on the floor would be so much easier to
avoid these kinds of circumstances.
- Yvette felt that she might get in trouble because of the incident.
- She has reassured me that now that she knows about the safety precaution of these residents, she
will not do it again.

Steps to be taken by Employee:

- Discussed with Yvette and to other care staff members on 03.01.2023 during handover that care
staff should always think about resident’s safety. Especially, when they know that resident is unable
to use the buzzer or have no means to call for help. Discussed with them MS has paralysis and his
mobility is poor leaving him in such a situation will put him at risk of falling.
- Reiterated to the care staff that if they are not sure of what they are doing they should read the
care plans or asked senior staff member on how to manage residents.
- To include in the handover the resident of the day and discuss about what they know about the
resident so the new care staff will know about them.
- Emphases on the assignment of the keyworker among the team.

Support Required from Employer

Reassurance given that she will not be in trouble and the supervision is just to help her get through her
transition. Yvette has reassurance me that she will not do it again and will continue to prioritize resident’s
safety all the time.
