That, the above-mentioned firearm/s is listed under investigation (UI) category in the
Firearms Information Management System (FIMS) of the Firearms and Explosives Office (FEO)
of the Civil Security Group (CSG);

That, I am requesting to the lifting of Forty Eight (48) Firearm/s mentioned Under
Investigation (UI) category that was deposited at FEO after it was ordered by SOSIA to Cease to
Operate (CTO) for failure of its renewal of License to Operate (LTO) within the prescribed
period;

That, I am hereby taking under oath that the above listed firearm/s are not involved in
any pending case.

That, upon the lifting and processing of its renewal, I will furnish copy of the said
renewed firearm/s to the office of the Inspection and Enforcement Section, FEO within fifteen
(15) days from the date it was lifted;

That, if I failed to process and renew my license in the given time. The Office, of the
Inspection and Enforcement Management Division, FEO will again place my firearm/s in the list
of Under Investigation (UI) category at the FIMS, FEO;

That, I undertake this affidavit of undertaking to attest the truth of the foregoing.
