Course Description: This course deals with the concepts, principles, theories, techniques in
the provision of basic care on terms of health promotion, disease
prevention, restoration, maintenance and rehabilitation at the individual
and family level. It include the study of the Philippine Health Care Delivery
System, national health situation and the global context pf public health.
The learners are expected to provide safe appropriate and holistic nursing
to individual and family as clients in the community setting utilizing the
nursing process

accordance with existing laws, legal, ethical and moral principles for safe nursing practice

Field Health Service Information System ( FHSIS)

It is the Recording and Reporting system of the DOH

• A basis for priority setting


• Planning and decision making at different levels ( barangay, municipality,
district, provincial, regional and national
• Monitoring and evaluating health program implementation

Objectives :
– To provide summary data on health service delivery and selected programs
accomplished indicators
– To provide data which when combined with data from other sources can be used
for program monitoring and evaluation purposes
– To provide standardized facility level data base which can be accessed for a more
in-depth studies
– To ensure that the data reported to the FHSIS are useful and accurate and are
disseminated in a timely manner and easy to use fashion
– To minimize the recording and reporting burden a the service delivery level, in
order to provide more patient care and promotive activities

The fundamental building blocks of the FHSIS
Contains the dates, names, address of the patients.

– It is the record of patients symptoms, complaints, diagnosis, treatments


and dates of encounter with the health provider

2. Target client’s Lists ( TCLs)


2nd building blocks
It is a facility based ledgers that records the services rendered to specific patients (
clients, targets, eligible)

❑ To carry out patient care and service delivery.
❑ Facilitate monitoring and supervision for services
❑ To report services delivered.
❑ To provide a clinic-level data base which can be accessed for further studies

Purposes of TCLs)
1. To help the health service provider

3. To report services delivered thus reducing the need to refer back to the ITR
accomplish recording

Data Entry Processing


The PHO is the processing node of the FHSIS.
It receives all the RFs from the rural health
Units (RHUs) of municipalities and from the City Health Offices (CHOs) of the component
cities in the province, and a designated Provincial Computer Operator (PCO) in the PHO
performs the data entry using DOH-developed software.
