STUDENT ABSENTEE FORM - (*For Madam Azni’s classes)

*Please fill out the details below and attach an official letter/MC (Medical Certificate)/Letter from your
Faculty or any other relevant document that can provide proof/details/reasons for being absent from
class.

*Students must fill out ONE form per absence (unless it is for prolonged MC’s that are more than 2
days). MC’s or letters must be sent in as an attachment along with this form, as proof of absence for
that particular day & week (when you were absent).

*Please send in your completed form and letter/document(s) as attachments, through MS TEAMS Chat
