    (Name of company) do herby solemnly declare that I/we have subscribed as
    shareholder/subscriber/elected/appointed as Director and have submitted necessary
    documents with the Company Registration office (CRO), (name of CRO) for its
    registration. In case my/our name is not security cleared by the Ministry of interior
    (MOI), I/we shall take immediate steps for replacement as
    director/shareholder/subscriber and/or shall transfer my/our shares to other person/s
    and shall file revised documents accordingly with the CRO, forthwith.
