CREATING THE BARANGAY COMMUNITY HEALTH TEAM (BCHT) OF
BARANGAY_____________________________ILOCOS NORTE

WHEREAS, Administrative Order No. 2010-0036 entitled, “The Aquino Health


Agenda: Achieving Universal Health Care for Filipinos” provides the strategic thrust to carry out
the Universal Care or “Kalusugan Pangkalahatan” (KP) for the poor families.

WHEREAS, Joint Memorandum Circular No. 2011- of the Department of Health


(DOH), Department of Social Welfare and Development (DSWD), Department of the Interior
and Local Government (DILG), and Department of Education (DepEd) mandates Local
Government Units (LGUs) for the creation and mobilization of a Technical Community Health
Teams (CHT) at the local communities to converge efforts in responding the health needs of the
populace especially the poor;
WHEREAS, DILG Memorandum Circular No. 2011-145 enjoins local chief executives
of all Local Government Units in coordination with the Department of Health to provide support
to KP-CHT mobilization-related activities;

NOW THEREFORE, I,_______________________, Punong Barangay,


Barangay_____________________, ILOCOS Norte, by virtue of the powers in me vested by
law, do hereby ORDER:

Section 1. Creation and Composition. The Barangay Community Health Team (BCHT)
of Barangay__________, ___________, Ilocos Norte shall be created with the following
composition:

Vice Chairman: Kagawad, Comm. Chairman on Health
Members:
Brgy. Health Workers (BHW) President
Brgy. Nutrition Scholar President
Brgy. Service Point Officer President
Midwife assigned in the Barangay
Parent Leader

CHT Partner:
Member/s Barangay Health Workers
Brgy. Nutrition Scholars
Brgy. Service Point Officers

Setion 2. Duties and Functions.

2.1 The Brgy. CHT Management Committee shall have the following duties and
functions:

1.Assist families in assessing and acting on health needs through health use planning;
2.Provide information on available health services in the locality;
3.Provide basic public health services such as among others, nutrition counseling, family
planning and responsible parenthood, antenatal care and follow up care;
4.Track families and their members for utilization of public health services;
5.Regular follow-up of beneficiary families in 4Ps areas; and
6.Facilite the organization of transportation and communication systems, outreach
services and linkages with other providers in the service delivery network;

2.2 The Brgy. CHT Partner shall have the following duties and function:

1. Asses health needs of families especially women, mothers and children and assist
mothers to fill up health plans to respond to families’ health needs and health risk;
2. Actively master list women of reproductive age especially those with unmet need for
family planning, women who are pregnant of post partum and children 0-11 months
old and those 6-59 months old;
3. Inform families of available services in the primary care facilities in the area;
4. Inform the families of the benefits and entitlements of PhilHealth enrollment;
5. Provide the twelve (12) key health messages using flashcards and family information
kits;
6. Advocates pre natal care, facility based deliveries, post partum and newborn care,
birth spacing, exclusive breast feeding, immunization and micronutrient
supplementation as provided in the key message
7. Report maternal and neonatal deaths to the RHUs and participate in maternal death
reviews;
8. Track and follow up clients in the particularly those who were scheduled for follow
up visits at the health facility assigned to them;
9. Report to their supervisors on a regular basis at least every week or until all
households are covered, their accomplishments on the number of households reached
using the recording and reporting forms.

Section 3. Effectivity. This order shall take effect immediately.
