An 11-year-old male patient presented with puffy eyes, headache, lethargy, a heart rate of 130 bpm, respiratory rate of 30 cpm, temperature of 37°C, and blood pressure of 200/100. A urine exam showed protein+, blood++, ketones-, white blood cells 2-4, red blood cell casts many, epithelial cells 0-1. Labs were otherwise normal except for a low C3 level. These findings indicate nephrotic syndrome. If the child develops pulmonary edema in addition to hypertension, diuretics like furosemide would be the best single treatment to address both issues.

21- An 11-year-old male patient developed puffy eyes, headache& lethargy. His HR is 130 b/m, RR 30 c/m, temp 37, BP 200/100. His urine exam is: protein +, blood ++. Ketones _ , WBC 2-4, RBC casts many, epithelial cells 0-1, CBC normal , LFT, KFT& electrolytes are normal. C3 is low. These data indicate: A. Ig A nephropathy B. Acute renal failure C. Nephrotic syndrome D. Post streptococcal glomerulonephritis
22- If this child has pulmonary edema in addition to
hypertension, the best single treatment of both his high blood pressure & distress is: A. ACE inhibitor B. Beta blocker C. Calcium channel blocker D. Diuretics like furosemide
