Internship/On-The-Job Training (OJT)

Agreement and Liability Waiver
I, Kristina Cassandra Dizon, a student of the College of Business and Financial Science of the University of Makati (UMak) taking up Bachelor of Science in Business

Administration Major in Marketing Management hereby voluntarily agree to undergo On-The-Job Training/Internship at City Government of Makati under the following terms
and conditions:
a. That the OJT/Internship or DUES training is an academic requirement for graduation;
b. That I am fully aware that my admission to the host training establishment (HTE) is purely as an OJT trainee/Intern and that NO EMPLOYER-EMPLOYEE relationship
exists between the HTE and myself;
c. That as an OJT Trainee/Intern, I am not a bonafide employee of the HTE, and is therefore not entitled to any benefits enjoyed by the regular employees of the HTE;

d. That I shall abide by the rules and regulations of both UMak and the HTE;

e. That I shall behave and show proper decorum at all times in accordance with the standards set by UMak and the HTE;

f. That I shall exercise care and due diligence in any task assigned to me under the training program;

g. That I shall hold all information, documentation, data or know-how disclosed to me in the course of my training program in confidence, and shall not disclose to any third
party or use the said information or any part thereof, even after the termination of my internship, without the prior written approval of UMak and the HTE;

h. That in the event that I receive personal information during my training program as defined in Data Privacy Act of 2012 of the Republic of the Philippines, I shall ensure
full compliance with the provisions of the Act and that such personal information shall only be used in the performance of my internship tasks;

i. That I shall not in any way hold the HTE responsible/liable from any sickness or injury that I may suffer in the course of my training;

j. That I shall be responsible for any and all damage/s to property or injury to third persons resulting from my intentional or negligent acts while undergoing training;

k. That the moment I am terminated with cause by the HTE, it shall mean my termination from the program where I am presently enrolled, unless, otherwise I can prove
that my termination from the HTE is unilateral, baseless, and devoid of any merit;

l. That violation of any provisions of this Agreement and other policies governing On-The-Job Training/Internship shall mean automatic termination from the program.
