As it has been socialized during WLI HSE meeting level 4 and barge safety talk about the HIPO NMI on
WLI couple weeks a go as describes below : (in bold red color)
“When lifting up 5.5” kill string + 1 joint tubing, suddenly side door elevator open and kill string + 1
joint tubing drop from 2 m height then hit tool basket.
There was no injured person”.
We’re taking this HIPO NMI very seriously due to the potential consequence are critical and may creates
major damages especially from Fact Finding of Investigation reported state this type of Side Door
Elevator was not equipped with a safety pin (or by said it by Verification Pin).  At the end  we would like
to highlight this Incident becomes a Lesson Learn for all of us which perhaps may useful and can taken as
simple guidance of Side Door Elevator as Pipe Handling Equipment in WLI Operations in the future.

Failures of manually operated side-door elevators

Introduction:
 There have been several serious dropped object incidents in such as attached above which involving the
use of manually operated side-door elevators to lift diameter well casing. The incidents involved the
unplanned opening of the elevators, during tailing-out or lifting of pipe from a horizontal position,
allowing the load to fall. Investigation of the incidents has raised concerns about the safety of the
latching mechanism and elevator condition thus the operating handheld manual.

We are as Users of the equipment side door elevator should ensure that:

 the use of such equipment is fully risk assessed;

 the equipment is maintained in an efficient state, in efficient working order and in good repair, with
its maintenance log kept up to date;
 its use is provided with suitable supervision and control measure to ensure the elevators are properly
latched and secured before any lift commences;
 those using or supervising the use of the equipment are provided with information, instruction and
training on the use of the equipment, including any additional control measures;
 safety management systems are in place to check compliance with the above.

A Side Door elevator is constructed in 2 main parts, a body and a door. When the elevator is closed, the
latch grips around the lug which is part of the body. The latch lock assures the latch is properly locked. In
order to validate the latch & latch lock are fully engaged, a verification pin must be fully installed. Only
then load should be transferred to the side door elevator.

MAINTENANCE & INSPECTION based on API RP 8B

If cracks, excessive wear etc or missing safety / securing part are recognized, please don’t be used and
inform an authorized service company to repair and or install the devices immediate. A regular
preventative maintenance program should be established for all elevators. Any written maintenance
procedures should be given to the crew or maintenance personnel (based on manufacturer instructions)

Generic Maintenances :

Daily lubrication
Grease point 1: Hinge Pin
Grease point 2: Latch Pin
Grease point 3: Springs

Daily Inspection
Inspect visually:
Latch Opening Arrangement. Presence of all bolts, nuts, safety elements, lock wire. Check for wear and
bending of verification pin repair.

Daily Test
Function the elevator daily. If any damage or malfunction were found, take the elevator out of service
for repair  

Locking of screws
All Screws are normally secured by a mechanical bolt lock or with a safety wire. All other screws are
secured by metal adhesive (Lock tite).
Ensure the correct retention method is applied.

Latch and latch lock

Ensure that the latch and latch lock mechanism are functioning properly. Hinge pins, latch lug surface
and link contact surface should be lubricated.

Generic Inspection :

For generic purpose may adopted the checklist below :

General Operational Safety issues :

1.  Never open the elevator when the pipe load is still suspended by the elevator.
2. All warning plates, sign s an d labels attached to the equipment must be observed.
3. Do never unlatch/open the elevator while a pipe is suspended in the elevator; the pipe will be
lost!
4. While using the elevator, always make sure the door is completely closed with the latch/latch
lock fully engaged an d the verification pin properly in stalled.
5. Never use the elevator without verification pin . This can create serious incidents.
6. Do not close the door with the verification pin in place. This will damage the pin and the door
will not close completely. Always close the door first and thereafter place the verification pin .
7. Pay special attention to the verification pin for any sign s of wear , bending or damage at any
time. In case the pin is damaged, replace immediately by a new, original one.
8. Using the elevator with damaged or worn parts can create serious incidents.
9. The picking up of horizontal or tilted pipes is dangerous. This use is permitted but with serious
analyze. Due to the danger it should be performed by license operating personnel.
10. Applications are only valid when the elevator is in good condition, does not have excessive
wear , cracks or other defects, or previous weld repair and has not been misused.
11. Failure to conduct routine maintenance could result in equipment damage or injury to
personnel.
12. Wear personal protection equipment while working with the equipment.
13. Stay away from the elevator in case it is provided with a rotation system. It may rotate forward
and backward without warning .
