AFFIDAVIT

That I, MARTHY P. MEDDRANO, of legal age, married, Filipino and a

resident of Hyacinth St., cor. Gumamela St., Banezville 2, Fatima Village,
Tacloban City, after having been duly sworn to in accordance with law do
hereby depose and say;

1. That I am a proprietor of ANDREA’S PIZZA with business address at

Hyacinth St., cor. Gumamela St., Banezville 2, Fatima Village, Tacloban City;

2. That relative thereto, I am applying for a business permit at

TACLOBAN CITY LICENSING DIVISION;

3. That there is no objection from the immediate neighbors of the

above-mentioned property where the proposed business is located;

4. That I am executing this affidavit in attestation of the veracity of the

foregoing facts and statements and in compliance with requirements of
issuance of business permit.
