BASIC JOB STEPS   POTENTIAL HAZARDS RECOMMENDATIONS
A. AREA PREPARATION (WORK AT HEIGHTS)

1. Scaffold erection (mobile scaffold) 1.1 Unsafe scaffolding 1.1.1 Scaffold must be erect as per standard/ client
requirements
1.1.2 Scaffolding Foreman must prepare a STARRT
Form for the erection of 15 meters high.

1.2 Hazard to fall 1.2.1. All Scaffolders should wear full body harness
equipped with two (2) LANYARD (inspected and
approved “colour coded”)
1.2.2. Ladder Arrangement (Access going to the top)

General Instruction:
More than two (2) meters high work must comply with the
standard – Full body harness with shock absorber
attached is required for more than four (4) meters.

2. Man-Lift (Elevating Work Platform) 2.1. Defective equipment 2.1.1. Approved inspection sticker (TAL-ID)
2.1.2. STARRT Form to be completed prior to
commencing
2.1.3. Full body harness with double lanyard and shock
absorber
2.1.4. Operator Certificate
2.1.5. Fire extinguisher (available in basket)
2.1.6. Third Party inspection
2.1.7. Barricade the working area

2.2. Environmental and health hazard due to spill 2.2.1. Report all environmental spill to GAMA-HSE Dept.
of diesel, oil, fluids.
3. Man Lift (Man Basket) 3.1. Hazard to fall 3.1.1. TAL-Work Permit is required
3.1.2. Workers to be trained in the use of the Full Body
Safety Harness (FPT) prior to commencing work
3.1.3. Third Party inspection and certificate
3.1.4. Restriction (close barricades) of the working area
3.1.5. All materials and tools that will be use at heights
should be properly secure and tied with a rope
3.1.6. Two Tag lines (length should depend on the
heights of work)
3.1.7. Provide additional worker to hold the tag lines
3.1.8. Supervising requires

B. CONCRETE PREPARATION

1. High elevation Preparation 1.1 Hazard to fall due to open platform 1.1.1. Fall Protection Training (FPT) requires
1.1.2. Make sure that scaffold is inspected and tag and
weekly inspected
1.1.3. Ensure the 100% tie off (double lanyard)
1.1.4. Ladder Arrangement (Access going to the top)

General Instruction:
-Only Scaffolders are allowed to alter or rectify
scaffolding “Do not alter erected scaffolding)

2. Use of mobile crane 2.1 Material fall (Human injury)


2.1.1 Ensure presence of trained riggers well identified on
the jobsite, use of tag lines to control load
2.1.2 Check wind speed is acceptable for lifting conditions
2.1.3 Position the crane in order not to hit adjacent
buildings / equipment
2.1.4 Safe working load of the crane shall not be
exceeded during any lift
2.1.5 Use colour coded & inspected lifting gear
2.1.6 Restriction (close barricades) of the working area

General Instruction:
DO NOT WORK UNDER A SUSPENDED LOAD
Same comments as (1.1.1 to1.1.6)

3. Bring up materials and tools above the working 3.1. Material fall (Human Injury) 3.1.1. No materials on hand when climbing the ladder
platform 3.1.2. Use rope to bring up material
3.1.3. Barricade the area

C. PREPARATION / INSPECTIONS PROVIDE / ACQUIRE APPROVED METHOD


STATEMENT AND APPROVED JOB SAFETY ANALYSIS
1. Pre-Commencement Activities (QA-QC 1.1 Key Personnel fell from High elevation 1.1.1 Fall Protection Training (FPT) requires

D. WORKING AT HEIGHTS

1.2 Hazard to fall 1.2.3. All Scaffolders should wear full body harness

1.2.5. Casters wheel always be lock (mobile scaffold)
1.2.6. No riders allowed whilst moving (mobile scaffold)
