Flags
Yellow flags are psychosocial factors including a previous history of anxiety and depression,
impending compensation, absence from work, sickness benefit, invalidity benefit, passivity and
high levels of dependency and poor coping skills.
Red flags are clinical features that should alert the therapist to the possibility of severe
pathology. They include bladder and bowel malfunction, saddle anesthesia, bilateral
paraesthesia, neurological signs, unexplained weight loss, a past history of carcinoma, general
debility and fever.

Antalgic gait: A protective gait pattern where the involved step length is decreased in order to
avoid weight bearing on the involved side usually secondary to pain.
Ataxic gait: A gait pattern characterized by staggering and unsteadiness. There is usually a wide
base of support and movements are exaggerated.
Cerebellar gait: A staggering gait pattern seen in cerebellar disease.
Double Step gait: A gait pattern in which alternate steps of a different length or at a different
rate.
Equine gait: A gait pattern characterized by high steps, usually involves excessive activity of the
gastrocnemius.
Festinating gait: A gait pattern where a patient walks on toes as though pushed. It starts slowly,
increases, and may continue until the patient grasps an object in order to stop.
Hemiplegic gait: A gait pattern in which patient abduct the paralyzed limb, swing it around, and
bring it forward so the foot comes to the ground in front of them.
Parkinsonian gait: A gait pattern in marked by increased forward flexion of the trunk and knees,
gait is shuffling with quick and small steps, festinating may occur.
Scissor gait: A gain pattern in which the legs cross mid-line upon advancement.
Spastic gait: A gait pattern with stiff movements, toes seeming to catch and drag, legs held
together, hip and knees slightly flexed. Commonly seen in spastic paraplegia.
Steppage gait: A gait pattern in which the feet and toes are lifted through hip and knee flexion to
excessive heights; usually secondary to dorsiflexor weakness. The foot will slap at initial contact
with the ground secondary to decreased control.
Tabetic gait: A high stepping ataxic gain in which the feet slap the ground.
Trendelenburg gait: A gait pattern that denotes gluteus medius weakness; excessive lateral trunk
flexion and weight shifting over the stance leg.
Vaulting gait: A gait pattern where the swing leg advances by compensating through the
combination of elevation of the pelvis and plantar flexion of the stance leg.
ERB'S PARALYSIS:
Site of injury: The region of the upper trunk of the brachial plexus is called Erb's point. Six
nerves meet here. Injury to the upper trunk causes Erb's Paralysis.
Causes of injury: Undue separation of the head from the shoulder, which is commonly
encountered in 1)birth injury 2) fall on shoulder, and 3)during anaesthesia
Nerve roots involved: Mainly C5 and partly C6.
Muscles paralyzed: Mainly biceps, deltoid, brachialis and brachioradialis. Partly supraspinatus,
infraspinatus and supinator
Deformity
Arm: Hangs by the side, it is adducted and medially rotated
Forearm: Extended and pronated
The deformity is known as "Policeman's tip hand" or "Porter's tip hand".
Disability:
Abduction and lateral rotation of the arm (shoulder).
Flexion and supination of forearm.
Biceps and supinator jerks are lost.
Sensations are lost over a small area over the lower part of the deltoid.

KLUMPKE'S PARALYSIS:
Site of injury: Lower trunk of the brachial plexus.
Cause of injury: Undue abduction of the arm, as in clutching something with the hand after a fall
from a height, or sometimes in birth injury.
Nerve roots involved: Mainly T1 and partly C8.
Muscles paralyzed:
Intrinsic muscles of the hand (T1)
Ulnar flexors of the wrist and fingers (C8).
Deformity: (position of the hand): claw hand due to the unopposed action of the long flexors and
extensors of the fingers. in a claw hand there is hyperextension at the metacarphalangeal joints
and flexion at the interphalangeal joints.
Disability:
Claw hand
Cutaneous anaesthesia and analgesia in a narrow zone along the ulnar border of the forearm
and hand.
Horner's syndrome: ptosis, miosis, anhydrosis, enophthalmos and loss of ciliospinal reflex- may
be associated. This is because of injury to sympathetic fibres to the head and neck that leave
the spinal cord through nerve T1.
Vasomotor changes: The skin areas with sensory loss is warner due to arteriolar dilation. it is
also drier due to the absence of sweating as there is loss of sympathetic activity.
Tropic changes: Long standing case of paralysis leads to dry and scaly skin.The nails crack
easily with atrophy of the pulp of fingers.
