Provides client privacy and place client in semi-Fowler’s position if not contraindicated. e. Assess the client’s need for suctioning and possible contraindications 2. Prepare the equipment, test the patency and pressure of suction, lubricate it and introduce the catheter. a. Attach the Yankeur to the suction machine. Turn on the suction device on the proper negative pressure level. Suction level should be 100-150 mmHg (wall suction) or 10-15 mmHg (portable suction) for adults. b. Test suction’s function by covering the hole of Yankeur suction using the thumb to feel the pressure and suction up small amount of sterile water/normal saline solution. c. Moisten the tip with sterile water, saline, or water-soluble lubricant using the dominant hand. With your non-dominant hand, remove patient’s oxygen mask. If nasal prongs, it may not be removed anymore. Then, put towel on patient’s chest. d. Do not apply suction or pressure during the insertion to avoid irritation on mucous membrane. Pull the tongue forward using gauze if needed. 3. Perform suctioning a. Insert Yankeur about 10-15 cm (4-6 in.) along the gumline into the oropharynx, keeping the Yankeur moving. Be careful not to advance it to the throat to avoid gagging reflex. b. With gentle rotation of the catheter, apply your non-dominant hand to the control and start suctioning for at least 10-15 secs. c. Encourage client to cough to remove secretions from lower region of airways. 4. Repeat if necessary. a. With the prepared basin filled with sterile water, rinse and flush the catheter. Relubricate and allow client for ventilation and replacement of oxygenation. Limit only the suctioning for 5 minutes. 5. Reassess client’s respiratory status (O2 sat, breath sounds & respiratory assessment). Place client in comfortable position. Dispose all the equipment used and perform hand hygiene
