I am an honest customer of your bank and having a savings account in your bank branch for
past 9 years. Through this letter I would like to request you to issue a new ATM Visa card to
me.

I already had an ATM card previously, but it has been brooked due to the change of new ATM
Cards which your bank introduced. Hence, I would like to request you to issue me a new
ATM debit card and since my nearest NATSAVE bank branch is in Solwezi am requesting
your good bank to send it here and am authorizing it be sent to Solwezi NATSAVE Bank.

I would once again request you to issue me a new card at the earliest. If there are any charges
