AFFIDAVIT OF PARENTAL CONSENT

We, VITALIANO INTE and DELIA INTE, both Filipino, both of legal age, married, and a
resident of Bayawahan, Sevilla, Bohol, after having been duly sworn to in accordance with law,
hereby voluntarily depose and say that:

1. We declare under the penalty of law the following:

a. We are the lawful parents of MAREX A. INTE who was born on April 1, 2004 at
Bayawahan, Sevilla, Bohol,;
b. We are aware that MAREX A. INTE is a candidate of a special enlistment for
the Airforce of the Philippines;
c. We allow our son to undergo the necessary physical and/or mental examination
to be conducted by the Airforce of the Philippines located at Mactan–Benito
Ebuen Air Base;
d. We hold the Airforce of the Philippines and any of its military and civilian
personnel free and harmless from any liability whatsoever for the death, injuries,
loss or damaged caused by any fortuitous events/or force majure, act of the
public enemy, act or omission of the candidate beyond the lawfully authorized
scope of activities by the Airforce of the Philippines, and other analogous
circumstances.
2. We warrant under the law that we are both lawfully authorized to sign this document;
3. We undertake to indemnify the Airforce of the Philippines for whatever damage incurred
should any entries that we provide in this document be found falsified;
4. We execute this document to attest to the truth of all foregoing facts, and for any other
lawful intent and purpose it may serve.
