Diagnosis: Bronchial Asthma

I. General Objectives

To provide continuity of care to N.P. at home in the absence of a health care provider.

II. Specific Objectives

1. Take medications at the right dosage, amount, time and route.


2. Explain the importance of hygienic cares.
3. Discuss the risk factors and clinical manifestations of asthma.
4. Develop a dietary or nutrition chart to have proper intake and stict monitoring of foods
that needs to be avoided.
5. Discuss the importance of adequate nutrition and simple exercises in the promotion of
physical and mental wellness.

III. Health Teachings

A. Knowledge
a. Explain to the patient what are the possible side effects in the given medications.
b. Discuss to them the implications of several common allergies on the human
body.
c. Describe many common asthma treatment techniques or inhaler techniques.
d. Generate a list of several symptoms of Asthma such as breathing, wheezing,
chest tightness for them to have awareness.
e. Discuss to them the importance of having an “Asthma Emergency Plan”.
f. Explain to them the frequency and severity of asthma such as the “Intermittent
Asthma” and “Persistent Asthma”

B. Personal Hygiene
a. Discuss to the patient how personal cleanliness is important to develop our
immune system.
b. Explain to the patient harmful effects of indoor air pollution and how to prevent
this pollution at home.
c. Emphasize to avoid using dirty or stinky clothes.
d. Emphasize the patient’s cleanliness in the home especially in changing the bed
linens every week.
e. Emphasize to the patient to perform hand wash hygiene and to cover your
cough/sneezes.

a. Instruct client to have simple daily exercises or exercises that are appropriate to
his age and capacity.
b. Tell the patient to do warm-up activities and have a cool-down time
afterward activity.
c. Introduced to the patient the different breathing exercises to improve breathing
and quality of life.
d. Tell the patient to avoid doing outdoor activities during cold seasons.
e. Encourage patient to include in her athma plan yoga’s for asthma relief.
f. Encourage the patient’s family to not provoke the patient’s emotions that can
result in an asthma attack.
g. Encourage patient to practice being at ease in all instances.
h. Instruct patient to sleep in supine position or elevate the upper body.

D. Nutritional Instructions
a. Instruct patient to avoid drinking cold water after doing daily physical activities.
b. Emphasize eating right and maintaining healthy diet.
c. Avoid drinks / foods with caffeine and other foods such as dried fruits and food
allergies.
d. Encourage the patient to stop smoking or using tobacco since he is now taking
medications.

E. Medication
a. Instruct patient to take:
i. Prednisone 5 mg/tab 1 tab 2x a day for 3 more days
ii. Azithromycin 500 mg 1 tab once a day for 5 more days
iii. Seritide inhaler 2 puffs every 12 hours (7AM-7PM) to gargle after puffs
iv. Multivitamins 1 capsule once a day for 30 days
v. Atorvastatin 40 mg/tab 1 tab once a day at 8 PM for 30 days
b. Emphasize to patient strict compliance to medication.

F. Follow-up Visit
a. Remind client for a follow-up check-up at the clinic next week, June 9,2022.
b. Have the CBC after 5 days and repeat RT PCR test.
