Incident Reporting (Near Miss, Unsafe Condition and Unsafe Act)

Incident: An unplanned, undesired event that adversely affects completion of a task (OSHA)
If not corrected, a condition that led to a minor incident can next time cause a serious injury
Capturing less serious safety incidents & putting corrective actions in place avoids future severe injuries
Incident Reporting & Investigation are key risk reduction tools.

ISOLATION SWITCH 9G-2758
PDI (PRE- DELIVERY INSPECTION) WORK ORDERS

If not already fitted

 A 9G-2758 (or 433-1540, if there is not space
to fit 9G02758) is fitted
 The cost of 9G02758 is added (booked and
issued) to the PDI job and charged as part of
the PDI to commissioning account

NON-PDI WORK ORDERS (GENERAL WORKSHOP & FIELD)

If not already fitted

 The customer should be asked if he/she wishes to
purchase 9G-2758 as part of the work order
scope (this would be best asked/confirmed at the
time of the CIF-Customer Interview Form)
 Kerchanshe Serviceman fits the 9G-2758 Isolation
Switch he/she has been issued with the start of
the job.
 If the customer has agreed to purchase 9G02758
it is left on the machine and the Serviceman is
issued a replacement from stock (booked and
charged to the job)
 If the customer does not wish to purchase 9G-
2798, the Serviceman removes his own at the end
of the job and keeps it for the next job.
ADDITIONAL NOTES

In all instances, a picture of the installed isolation switch needs to be taken; during the PDI or
TA1 preceding the General Workshop or Field repair. This should be parts of the CAT inspect
report for PDI or TA1. (See photos)
