AFFIDAVIT OF LOSS
I, AVELINA S. CONCEPCION of legal age, Filipino, and a
resident of No. 3 Road 4, Pacheco Subd., Dalandanan, Valenzuela City,
after having been duly sworn to in accordance with law do hereby depose
and state:

1. That I am the owner of a Commercial Building/apartment under

BAN: 06101507 situated at 505 Mc-Arthur Hi-Way, Malanday,
Valenzuela City;

2. That the said commercial building was mortgaged in favor of PNB

Bank and eventually foreclosed in its favor. I have received notice
of eviction together with the other occupants of said building for
reason of the foreclosure in favor of PNB bank;

3. That as a result thereof, on May 30, 2016 we removed all our

things and other appliances and belongings including business
records and documents from the said establishment;

4. As a consequence thereof, I retired/closed my renting business on

July 4, 2016;

5. However, after the closure of my business and upon looking for

the business records and pertinent documents, I have found out
that the Books of Account, collection receipts, certificates of
registration and other pertinent documents of my commercial
building were missing, maybe they were misplaced during the
removal of all our belongings from the said establishment;

6. That I have exerted diligent efforts to locate the same but all our
efforts proved to be all in vain; and

7. That I am executing this affidavit to attest to the truth of the

foregoing statements and for all legal intent and purpose it may
serve;

IN WITNESS WHEREOF, I have hereunto set my hand this 23 rd

day of March 2017 at Malolos City, Philippines.
