Ms. Theresa Ozaeta

ICU Head Nurse

Makati Medical Center

2 Amorsolo Street, Legazpi Village,

Makati, 1229 Kalakhang Maynila

Dear Ms. Ozaeta,

I’m pleased to write you today regarding my application for the Nursing position at Makati
Medical Center. Your Chief Nursing Officer Ysabelle Ozaeta referred me to this opportunity. I
am a Registered Nurse with over 8 years of experience practicing in a community health clinic.
I’m confident that my clinical skills, healthcare knowledge, excellent patient care, experience in
treating different health issues and improving lives will make me a strong addition to your
organization. I believe my skills, qualifications, and experience make me an excellent candidate
for this job.

Throughout my career as a Registered Nurse, I have worked mainly in a community health

clinic where I treated patients of different ages, health issues, and backgrounds. This kind of
setting helps me to learn about different sicknesses, illnesses, and injuries and how to treat them.
With this experience, I have become all-around nursing professional.

In my current position at San Isidro Health Center, I have successfully led many
campaigns and projects for health awareness in our community, and hopefully, I can be able to
continue and expand these projects and campaigns with the help of your company. The
successful campaigns and projects I led includes:
• Aim for a Healthy Weight which provides help to people(including children) to
learn about reaching and maintaining a healthy weight and reducing the chance of
having diseases such as obesity, heart disease, diabetes, and high blood pressure.
• Stay in Circulation campaign that spread awareness about heart diseases, stroke,
and blood-related issues.
• The “Mind is Aging” project which helps other health professionals,
patients(specifically the old ones), and their families to know more about mental
diseases like Alzheimer’s disease, and help them in their needs. and;
• “Smoke to Stroke” project that targets teens and other patients who smoke to know
the different effects of cigarettes on their health. This project also helps the
community to be smoke-free.

In 2001, I graduated from the University of Sto. Tomas in Manila as a salutatorian and
have a Master’s Degree in Nursing for 7 years of proven experience effectively and efficiently
managing both patients and staff. I have also received tons of certificates, medals, and awards
including the Nursing Mentor Award in 2010, Nursing Practice Excellence Award in 2014,
Rising Star Award in 2016, LPN Excellence Award in 2017, Nursing Professional Development
Excellence Award in 2020, and Nursing Excellence in Research and Innovation Award this
2022. But behind these certificates, medals, and awards that I have received. My passion,
dedication, hard work, love for my career and for my patients is what I’m most proud of.

Though I have greatly enjoyed working at San Isidro Health Center, I feel that I’m ready
to take a new step and challenge with your company. It will be an honor for me to be part of your
well-known organization as I continue my legacy, platforms, and projects in health care. I
believe your organization and company will also help me to grow not just as a nurse but also as
an individual. I’m excited and ready to share with you my learnings and skills and learn more
about different health issues, health care, and systems. I look forward to hearing from you and
would love and be deeply honored to present and explain further my skills during an interview.
Please see the attached resume with this letter.

Sincerely,

Ana Carmela Mortel
